London Cares is a not-for-profit agency aimed at building relationships and supporting those who are experiencing homelessness making it possible to help move some of the most vulnerable people in our community from street to home. Through a low barrier, harm reduction compassionate approach we build relationships and are advocates for those we work with to build bridges and connections to the resources they need in support of their success and well-being.

London Cares programming continues to grow and expand to meet the needs of the community we serve. We are seeking Housing Stability Workers to join our team in a full-time capacity working 35 hours per week. In order to provide 24/7 support there will be day shifts, afternoon shifts and a consistent overnight team. Your shifts will be approximately 8 hours in length and will include weekdays, weekends and statutory holidays - shift schedule to be determined. This program will have a minimum of 2 staff on site 24 hours a day, 7 days a week.

This NEW Highly Supportive Housing Program is in partnership with London Health Science Centre (LHSC) and has been developed to support high acuity individuals who are living on the streets and have high emergency department and hospital use. This program will divert high acuity individuals out of emergency departments and into long term permanent supportive housing. Through a low barrier, trauma informed, harm reduction lens supports are wrapped around individuals through a multi -disciplinary care team to provide tailored care plans unique to each individual. This housing initiative prioritizes individuals who are living with tri morbidity (complex mental health, substance use concerns and complex medical needs) with the intention of having a multi disciplinary team on site to provide wrap around support to all individuals.

RESPONSIBILITIES

In this role you will be a member of the Supportive Housing Team reporting to the Supportive Housing Coordinator, or any other person as the Employer may assign from time to time and carry out such duties as assigned to you by the Employer.

Duties include, but are not limited to:

- Establish and maintain relationships with participants who have experienced homelessness through the delivery of in-home intensive, ongoing and collaborative case management services.
- Provide intensive case management supports to a consistent caseload where you will be the primary housing stability worker.
- Working in a team environment, collaborate with other Housing Stability Workers to provide additional case management support when needed

Using a strengths based approach, support individuals and families to achieve stability in their home and community utilizing case management tools such as crisis plans, honest monthly budgets and guest management plans
- Help them to establish personal goals and support them to achieve those outcomes through regular communication, organizing meaningful daily activities, connecting them with other services (healthcare, addiction, trauma, employment, social, etc.), and creating proactive crisis support plans
- Foster a sense of community and belonging in a residential setting
- Assist with various tasks associated with transitions of residency such as orientation of new tenants
- Listen and respond to residents concerns, questions and requests
- Support with crisis management, de-escalation and conflict resolution in a team approach
- Teach and model life skills such as neighbour relations, cooking, cleaning, personal hygiene
- Respond to overdoses and monitor individuals who may be under the influence of various substances
- Perform other related activities, as necessary or as assigned

Qualifications:

- Post-Secondary education in the field of social work, human services, addictions, or mental health and/or equivalent experiences
- 2+ years of experience working with individuals experiencing homelessness and vulnerable populations
- Complex case management experience
- Clear vulnerable police record check
- Valid Standard First Aid/CPR is a requirement of this position.
- A valid ‘G’ drivers license
- Training in non-violent crisis intervention and ASSIST considered an asset

Skills and Abilities:

- Comfortable with supporting participants in a residential living environment
- Advanced skills in motivational interviewing and facilitating SMART goals
- Experience working with multi disciplinary teams such as health care, housing, developmental and other community service agencies
- Ability to cope with and embrace change, ambiguity, and uncertainty
- A willingness to uphold a low-barrier approach and understanding of harm reduction and housing first principles.
