Care Navigation Highlights:_**
- _ Provides on-going case management and support to clients/caregivers._
- _ Conducts home visits for initial, assessment and care planning for each client on assigned caseload._
- _ Assists with scheduling and planning of support services, as well as connecting clients to external resources, as appropriate._
- _ Connect clients and caregivers to appropriate community supports/resources._
- _ Provides assistance with navigating the health care system._
- _ Provides crisis intervention and support (CIS) in critical situations and monitors client status._
- _ Collaborates with partners and represents the agency externally at planning meetings._

Position Requirements:_**
- _ Regulated Health Care Professional with Registration as a health or social work professional, including registered nurse, physiotherapist, occupational therapist, speech language pathologist, or social worker. Registered Social Worker is an asset._
- _ A minimum of one year of experience within the community support service sector or health care environment._
- _ Have experience working with the frail and elderly population, and knowledge of community resources including funding sources._
- _ Demonstrate case management/coordination experience including assessment, scheduling, and implementation of services, care planning, plan monitoring, and evaluation processes._
- _ Understand in-home health care issues, and familiarity with the Assisted Living Services for High Risk Seniors Policy._
- _ Have excellent computer skills with proficiency in MS Word, Outlook and Power Point; and knowledge of RAI-CHA is an asset._
- _ Have a Valid Ontario Driver’s License and access to a reliable vehicle_
- _ Valid Vulnerable Police Record check completed within last 12 months._
- _ Current CPR/First Aid Certification._
- _ Fluency in other language(s) is an asset._
- _ This position requires you to be fully vaccinated against COVID-19 and you must provide Lumacare with proof of vaccination prior to employment._
