**About the C.A.R.E. Program [Coordinated, Accessible, Respectful, Equitable]**

The Central West Ontario Health Team (CW OHT) Integrated Home-Based CARE Program aims to provide wrap-around interdisciplinary care to reduce Emergency Department (ED) utilization for older adults experiencing frailty. Modelled after successful programs in Toronto Central (House Calls) and the East York Region North Durham OHT (Seniors Home Support Program), this program will accept referrals from ED/acute care, community, primary care, and home care using bestpractice tools to identify frailty and those at high risk for delayed discharge from hospital due to Alternate Level of Care (ALC) designation. The program involves comprehensive assessment provided by support staff and home visiting Care of the Elderly (CoE) Physician to develop a shared care plan and achieve patient and family/ care partner focused goals to address social, medical, and cognitive frailty.

Queen Square Family Health Team (QSFHT) is looking for a passionate Administrative Assistant to support daily operations and administrative duties for the CARE Program. This position requires excellent administrative skills, ability to work independently, and strong interpersonal communication skills to engage with patients, their families and/or caregivers, and other health care providers.

**Status**

Permanent part time, up to 19 hours/week distributed Mon to Fri per week

**Job type**

Remote/work from home with periodic in-person meetings with Program Manager and/or CARE team members

**Key Roles and Responsibilities**
- Gain familiarity and comfort with CARE program, patient flow, and administrative processes
- Answer a multi-line internet-based telephone system and redirect calls appropriately to CARE team members when needed
- Make phone calls to patients to provide information and book appointments with CARE team physicians and/or nurse based on careful coordination of schedule
- Use the organization’s electronic medical record (EMR) system - Arya EHR to: monitor incoming referrals, create patient profiles, manage appointments and provider schedules, complete assigned tasks related to patients
- Process correspondences, draft replies and perform other routine tasks
- Multi-task and be adaptable in an evolving environment
- Maintain privacy and confidentiality of all data and information
- Gain familiarity and understanding of other health care agencies and partners to strengthen communication and collaboration with CARE Program
- Participate in program planning, implementation, and evaluation activities
- Participate in team meetings
- Demonstrate teamwork and adherence to privacy and professional standards of practice
- Perform other duties as assigned by the Program Manager for efficient operations

**Qualifications**
- Post-secondary education and or/relevant education and a minimum of 2-5 years’ experience in a related field such as business administration
- Completion of a Medical Secretary Course (medical terminology, pharmacology) is an asset
- Experience in using EMR systems is an asset
- Proficiency with Windows 10, Microsoft Office Suite including Outlook and Teams, Videoconferencing tools such as Zoom, and conducting internet-based research queries
- Experience working as part of an interdisciplinary team is a preference
- Comfortable with electronic documentation and use of technology in the workplace
