**Overview**
The Wellness team at Niagara Naturopathic & Wellness Clinic includes clinic owner and Naturopathic doctor Jacquie Moulton, along with a Psychotherapist, Homeopathic consultant, Kinesiologist, & Hypnotherapist.
**Duties**
- Conduct assessments to determine the needs of children and families.
- Develop and implement individualized care plans based on assessment findings.
- Provide case management services, coordinating with healthcare providers, educators, and community resources.
- Assist families in navigating special education services and accessing necessary support.
- Utilize medical terminology to communicate effectively with healthcare professionals.
- Engage in data collection and research to improve service delivery and outcomes.
- Maintain accurate documentation of client interactions and progress.
- Advocate for patients' rights and facilitate access to appropriate resources.
- Participate in interdisciplinary team meetings to discuss patient care strategies.
**Qualifications**
- Bachelor’s degree in Social Work or a related field; Master’s degree preferred.
- Experience working with children in a clinical setting is highly desirable.
- Knowledge of special education processes and pediatric care practices.
- Strong skills in case management, including assessment, planning, implementation, and evaluation.
- Familiarity with medical terminology relevant to patient care.
- Excellent communication skills, both verbal and written.
- Ability to conduct research and analyze data effectively.
- Compassionate demeanor with a commitment to serving diverse populations.
- Strong organizational skills with attention to detail.
