**Position Summary**:
Foothills Country Hospice Society (FCHS) Program Coordinator supplements the implementation and ongoing maintenance of current/future FCHS programs in collaboration with the Social Work team and the Coordinator of Volunteer Services. This role has a focus on community engagement, volunteer supervision, program evaluation and reporting. This role aims to ensure the effective delivery of grief and community support programs, while also exploring opportunities for future programming based on community needs. The Program Coordinator will work closely with the hospice multidisciplinary team, practice in accordance with established policies, procedures, and standards, and ensure resident/client and family safety is a priority. Other duties as assigned by the Social Work & Grief Support Lead.

**Position Responsibilities Include**:
**Community Program Engagement**:

- Promotes the mission, vision, values, and goals of FCHS, while interacting with residents, families, employees, volunteers and the community.
- Utilize project coordination skills to create program schedules, implementation plans, risk assessments, and contingency plans.
- Promote and expand community connections to FCHS programs. Build relationships with community members to further promote FCHS programs and reputation.
- As head of the Community Support (Nav-CARE) program, meet with new community clients, provide program information, and match volunteers to clients, based on needs and personality profiles.
- Monitor and evaluate volunteer/client relationships to ensure positive outcomes for both.
- Collaborate with the Fund Development Team to engage in promotion of programs, facilitate grant reporting, and provide public updates on FCHS programs.
- Monitor and evaluate the provision of grief support and community support programming, making appropriate adjustments based on client feedback and response.
- Track program data to support program evaluation, reporting, and improvements in community engagement.
- Ensure that program expansion is delivered in alignment with any grant funding goals and expectations.
- Provide accurate and timely reporting, including program metrics and timelines, and support the Social Work and Grief Support Lead with relevant program information as needed.
- Optimize internal procedures and develop external processes to reduce barriers to accessing FCHS programs.
- Respond to requests and identify opportunities for education sessions or additional programs that meet community needs and give presentations when appropriate.
- Work alongside the Social Work team and Coordinator of Volunteer Services to develop and implement community-based programming that follows the ideology of the compassionate communities movement, striving to improve the health, wellbeing, and quality-of-life of those living in the Foothills Community.
- Share resources and learnings with other hospice societies interested in implementing community-based programs.

**Program Volunteer Coordination and Supervision**:

- Match volunteers with appropriate clients.
- Provide ongoing education and support for volunteers in the community programs.
- Host community volunteer meetings, debriefs, and/or connection sessions to foster a sense of community within that volunteer group.
- Evaluate volunteer and client satisfaction with community programming.

**Accountability**:

- Primary Reporting Line: Social Work & Grief Support Lead
- Secondary Reporting Line: Coordinator of Volunteers
- Supervisory Responsibilities: No
- Hours of Work: 32-40 hours week

**Preferred Minimum Requirements**:

- Post-secondary education in a related field or equivalent work experience may be considered.
- Experience in a non-profit organization is an asset.
- Experience with palliative and end of life care is considered an asset.
- Proven ability to manage, mentor and motivate a team in a collaborative environment
- Excellent interpersonal, oral, and written communications skills including facilitation and presentation skills.
- Proven ability to foster relationships.
- Excellent analytical reasoning and problem-solving skills.
- Advanced knowledge of Microsoft Office Professional software.

This position requires the individual to be compliant with all FCHS privacy and confidentiality policies and procedures; and to be compliant with all Privacy Legislation (FOIP, HIA, PIPA) relevant to FCHS.
