Detailed Overview:
The Clinical Educator, Counselling leads and collaborates with Primary Health Care leadership to plan, develop and implement education, orientation and training to other health professionals within the local Primary Care Network (PCN).
Provides clinical and educational support, plans, implements and evaluates educational activities and programs, and assists with the implementation of discipline competencies. As a clinical resource, reviews current clinical practice against evidence-based best practice and recommends changes, where required; supports complex or challenging referrals/discharges, collaborates with other healthcare providers, and gathers information for ongoing quality improvement and review.
This position may, as required, work at multiple locations within the community served by the local PCN.
**Responsibilities**:
- Determines education priorities that meet program and health professionals' learner needs; completes learning needs assessments of individuals or groups to assist in the determination of the educational requirements and their objectives.
- Develops, plans and revises educational and orientation activities and programs, including online learning, based on clinical evidence, policy (e.g. clinical decision support tools), literature and resources, and education theory.
- Creates supportive and inter professional learning environments; utilizes education theory and builds strong professional relationships with learners in order to facilitate open communication. Ensures sites are informed of educational resources and opportunities.
- Delivers learner-centered education to various health professionals, health care team members, and clients/caregivers through methods such as teaching, role modeling, demonstrating, mentoring, coaching, facilitating groups, providing lectures, and conducting formal presentations, including in both person and virtual formats.
- Plans and schedules educational activities and maintains a system for tracking attendance; maintains statistics for reporting purposes.
- Evaluates education effectiveness by measuring the outcomes, determining whether the clinical objectives have been achieved, and assessing whether learning has occurred that results in the demonstration of best practice.
- Serves as a Counselling clinical resource for complex or challenging clients or situations and supports complex referrals/discharges such as client behavioural challenges. Develops tools to enhance the exchange of information among staff and collaborates with clinical leaders and health care team members.
