We have the perfect Releif Full‑Time opportunity for a Clinical Pharmacist to join our team at Peace Arch Hospital located in White Rock, BC. This position is available until approximately November 2026 or until the return of the incumbent. Rotation Details 0630-1430 0800-1600 0830-1630 0900-1700 0800-1650 1010-1900 1100-1900 0740-1630 or as per operational needs. Current rotation is a combination of 7.5 and 8.3 hour shifts and one weekend every 6 weeks. Shift times are between the opening hours of 0630‑1900 Monday‑Friday and 0730‑1630 Saturday/Sunday. Key Services Individualized pharmacotherapy Counselling patients about their medication therapy Work in collaboration with physicians, and other health care providers to develop, implement, and monitor therapeutic plans Assist in the development of drug therapy guidelines for individual disease conditions Prepare and submit case studies and articles for publication in professional journals Acts as department resource person within your assigned area Participate in medication use evaluations, quality improvement initiatives and clinical drug trials Provides mentorship and supervision to pharmacy students, residents and new staff Ensure prescriptions are validated and dispensed accurately; address medication safety concerns and resolve discrepancies with the health care team Document professional practice activities proficiently Responsibilities Consult with physicians, nurses and other health care providers to develop therapeutic plans based on chart review and clinical assessment. Ensure appropriate documentation is maintained in the patient health record. Individualize patient medication therapy by referencing standard literature, reviewing patient charts and laboratory data, and modifying or developing therapeutic plans. Provide patient medication counselling, explaining drug therapy and use of equipment, and assess patient understanding. Assist the health care team in developing drug therapy guidelines for individual disease conditions by reviewing current literature and monitoring guideline effectiveness. Participate in ward rounds, program team meetings and unit-level pharmacy services to enhance patient care.\Act as department resource person in the assigned area of responsibility. Contribute case studies and review articles to professional journals. Provide instruction and supervision to pharmacy students and residents. Participate in quality improvement activities and audits, identifying and recommending changes. Perform drug distribution duties, interpreting medication orders and dispensing medications using a computerized system and ensuring legal and departmental standards are met. Review prescriptions for therapeutic suitability using standardized references and laboratory data. Communicate with physicians, patients and other health care team members for prescription clarification and problem resolution, documenting interactions. Ensure medications are accurately dispensed, compounded, labeled and that dispensing records are completed. Participate in medication use evaluation projects, maintain inventories, requisition drugs, monitor expiry dates, report procedural errors and support quality improvement activities. Participate in clinical drug trials, documenting associated materials and adverse reactions. Implement medication safety initiatives and quality control activities per established standards. Participate in staff meetings, committees and other programs as required, identifying improvement opportunities. Maintain records for narcotic and controlled drugs, special access program drugs and departmental statistics in compliance with legal and departmental standards. Prepare reports on activity, gathering data, accounting for variances and updating computerized files. Perform other related duties as assigned. Qualifications Current registration as a Pharmacist with the College of Pharmacists of British Columbia. Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training and experience. Skills And Abilities Broad knowledge of organization policies, procedures and standards of care. Knowledge of evidence‑based clinical pharmacy practice. Knowledge of medication use evaluation. Knowledge of research processes and methodology. Knowledge of drug distribution processes in a hospital setting. Ability to apply clinical pharmacy knowledge and make decisions with a minimum of supervision. Ability to communicate effectively both verbally and in writing. Ability to establish and maintain effective working relationships with other health practitioners, patients, family members and the public. Ability to organize and prioritize work. Ability to operate related equipment. Physical ability to carry out the duties of the position.
