Home and Community Care Support Services Champlain is one of 14 Home and Community Care Support Services organizations in Ontario. We are dedicated to ensuring our more than 64,000 patients annually receive the highest-quality health care and services at home. Our patient-care coordination teams help develop care plans for people of most ages, focused on maintaining each patient's independence and dignity. We also provide long-term care home placement services, and facilitate our patients’ access to the community-based services they need.

**Position Summary**:
The Registered Dietitian is responsible for the nutritional management of patients on their caseload. Primary responsibilities include the assessment of nutritional status/nutritional needs and the development, implementation and evaluation of care plans to meet these needs. The Registered Dietitian will collaborate with other health care providers and function as a key member of the interdisciplinary team

**Primary Responsibilities**:

- Provides a high standard of nutritional care for patients/residents through nutritional assessment, formulation of goals and recommendations, implementation and evaluation of care plans
- Accurately assesses, calculates and develops intervention of nutrition support
- Formulates plan of care and monitoring parameters using evidence-based guidelines for appropriate Nutrition Support Therapy. This includes enteral feedings and TPN (total parenteral nutrition)
- Collaborates with development of nutritional programs and educational materials ensuring they meet evidenced based recommendations
- Works as part of a multidisciplinary team to deliver excellent patient outcomes
- Monitors patients’ nutritional status, updates written records, and communicates effectively with the inter-disciplinary team members
- Makes recommendations for referrals for additional patient services and where appropriate, assist in facilitating
- Provides nutrition education/counseling to patients and caregivers
- Works closely with the team and other resources to develop new programs to meet identified patient needs
- Responsible for effective and efficient use of nutritional supplements. Reviews and updates nutritional supplement formulary as necessary
- Networks with internal and external stakeholders to promote new and existing programs
- Performs regular, timely charting, reporting and communication
- Records workload measurement data
- Attends staff meetings and case conference meetings, as required
- Other related duties may be assigned

**Education and Experience**:

- Bachelor’s Degree in Nutrition or Dietetics and completion of Dietetic Internship program recognized by the Dietitians of Canada
- Current Certificate of Registration from the College of Dietitians of Ontario
- Related professional experience which would normally be attained over a period of two years in a similar (Dietitian) role
- Experience working with a diverse caseload from infants to senior adults
- Related experience in a similar community-based setting is an asset
- Current CPR-C certification

**Knowledge**:

- Knowledge of health care system and the role of the Home and Community Care Support Services Champlain within the system
- Knowledge of Home and Community Care Support Services Champlain operations, quality, risk, and performance management principles
- Knowledge of relevant health care legislation
- Understanding of Home and Community Care Support Services Champlain partners and contracted service providers in the community
- Client centered, multi-disciplinary team approach to client care
- Proficient in the use of computerized processes including the use of mobile (laptop) technology

- Effective decision-making and problem-solving in crisis situations
- Ability to negotiate and implement an effective goal-oriented, client-centered care plan
- Team player with ability to accept and adapt to change
- Ability to maintain a positive attitude in stressful situations
- Ability to organize daily workload in the presence of frequent interruptions
- A valid Ontario Driver’s License or Provincial equivalent and access to a motor vehicle are required.
- Vulnerable Sector Check completed within the last six months

**Pay Rate**: Wages are in accordance with the collective agreement between Home and Community Care Support Services Champlain and OPSEU and starting salaries range from $40.409/hr. - $43.169/hr.

**Shift**: 3 shifts per week between from Monday to Friday, 830am-430pm

**Location**: 1100 Pembroke Street East, Pembroke OR 850 O’Brien Road, Renfrew

Home and Community Care Support Services Champlain offers flexible work options, this position will have a hybrid model of work with a combination of work locations, including community and clinical settings, patients’ homes, on-site, and remote.
