We are currently looking for an experienced and forward-thinking Care Coordinator to join our team. The Care Coordinator will provide outpatient/electronic outpatient case management for mental health conditions. As a Care Coordinator, you will be the first point of contact for our members and you will have an essential role in the patient care experience. You will be working as part of a group of highly qualified, social workers, therapists, doctors and allied health professionals. You will also play an essential role in the development and continuous improvement of the iHealthOX care experience.

Responsibilities include:
Assist in cases requiring additional non-clinical follow-up and care coordination, including, but not limited to, scheduling appointments, researching & answering plan benefit/claim questions, provider research, and obtaining medical records.
- Determine, develop, implement, and update the plan of care based on an accurate and comprehensive assessment of the member and current or proposed treatment plan.
- Conducts mental health screenings and assessments based on mental health check-ups.
- Work collaboratively with iHealth's team of Social Workers, Registered Nurses, and interdisciplinary teams to provide comprehensive case management and exceptional member experience.
- Anticipate potential barriers while establishing realistic, measurable member-centred goals & objectives and provide appropriate alternatives.
- Actively solicit member and family involvement to encourage goal completion.
- Work directly with members, vendors, and providers to ensure superior care coordination.
- Co-manage cases with nurses and social workers, ensuring all assigned tasks are completed on time for patients.
- Assist with organizing daily team huddles and meetings and ensuring follow-ups and action items are captured and assigned.
- Assist with overall program administrative duties including, but not limited to, scheduling meetings, maintaining meeting notes, developing materials, and client or internal reporting.
- Act as liaison between our Health Care Team and Member Advocate Team to allow teams to work with one another effectively.
- Evaluate Care Team workflows and develop new process improvement strategies to optimize efficiency and scalability
- Refine project management skills through collaboration with multiple teams, partners, and members
- Maintain strict confidentiality of sensitive materials and information at all times.
- Other duties and projects as assigned by the Clinical Director

You are a fit for the Care Coordinator position at iHealthOX if:

- You have experience providing care coordination, preferably within a healthcare or clinic setting and previous administrative experience
- You have customer service experience and experience in handling escalations.
- You are independent, tenacious, and capable of identifying and implementing timely and quality work.
- You are comfortable with ambiguity and open to collaborating cross-functionally.
- You have a solid moral compass and commitment to iHealth's core values of integrity, transparency, and trust.
- You are passionate, self-motivated, intellectually curious, and take ownership of everything you do.
- You are tech-savvy and can utilize or quickly learn SalesForce, SUMO Calendar, LiveChat and ZOOM.
- You have previous work experience working as a care coordinator, member advocate, or healthcare unit coordinator.

Education
- Bachelor's degree preferred Master level
- Graduate degree in counselling or Life Science
- Bachelors Degree in Nursing or more

Please note: This job is hybrid-working - employees are required to perform in-office shifts.
