**Overview**

**Responsibilities**
- Assess client needs and develop individualized plans in collaboration with the client
- Monitor client progress and adjust plans as necessary based on changing conditions.
- Serve as a liaison between client, families, and developmental/healthcare/mental health professionals to facilitate a high level of success
- Maintain accurate records and progress notes

**Skills & Education**
- Minimum Undergraduate degree in psychology, social work, sociology, or College Diploma in Developmental Services, Recreation and Leisure, or other related programmes
- Must be an independent thinker and work on their own but within a team approach
- Strong knowledge of people with an intellectual/developmental disabilities
- Excellent organizational skills with the ability to manage multiple cases simultaneously.
- Strong interpersonal skills to build rapport with clients, families, and team members.
- Ability to work collaboratively in an interdisciplinary team environment.
- Critical thinking skills to assess situations and make informed decisions

- Day shift
- Evening shift
- Monday to Friday
