Positive Living Niagara is a community-focused organization composed of passionate staff and volunteers. We are committed to providing support, education, and advocacy for individuals affected by or at risk of HIV, Hepatitis C, and other sexually transmitted blood-borne infections (STBBIs). We are dedicated to fostering inclusivity, equity, and the meaningful involvement of those we serve.
We are seeking a **Case Manager (Intensive)** who will provide a range of case management, planning, counselling and advocacy supports to HIV+ clients with complex needs. The Case Manager will provide support to individuals by coordinating services within the agency and in the community.
**Key Responsibilities**:
- Conduct initial assessments and develop Individualized Support Plans for clients within designated programs operated by Positive Living Niagara
- Facilitate referrals and system navigation to a range of community and agency supports, to maximize independence, health and quality of life.
- Transport clients as appropriate to accompany them and provide direct support.
- Collaborate and consult with other community providers and agency team members to ensure comprehensive care.
- Facilitate support groups and educational workshops on topics related to HIV, mental health, and harm reduction, as required.
- Respond to crisis and emergency situations.
- Prepare administrative and program reports for the funders and other bodies.
- Monitor and evaluate program activities to ensure optimal quality of service delivery to local communities.
- Maintain accurate and confidential client records in compliance with organizational policies and legal requirements.
- Stay informed about the latest developments in HIV care and harm reduction strategies.
- Participate in community outreach and advocacy efforts to raise awareness and reduce stigma.
- Other duties as assigned
**Qualifications**:
- Post secondary degree or diploma in social work, psychology or related discipline, with 4+ years experience in the field.
- Strong understanding of HIV, substance use, mental health issues and harm reduction principles.
- Excellent interpersonal and communication skills.
- Ability to work effectively both independently and as part of a multidisciplinary team.
- Commitment to cultural competency and sensitivity in working with diverse populations.
- Demonstrated organizational skills
- Leadership skills
- Knowledge of computer systems and software
- Accurate documenting, reporting and recording
- Presentation skills
- Valid G driver’s license in good standing with a reliable vehicle is a requirement
- French, Spanish and other language skills are assets
