**Social Worker (MSW)**
**Permanent Full Time**
As part of an interdisciplinary primary health care team, the social worker will use evidenced based care to provide assistance to clients and their families who cope with challenging situations, with a goal of helping improve and maximize their health and wellness.
Responsibilities include:
- Provide ongoing mental health counselling to registered clients and link with community resources and support as needed.
- Participate in the development and evaluation of individual and group education sessions
- Collaborate with interdisciplinary care team to ensure appropriate management and client follow-up as required
- Participate in community advocacy initiatives
**Qualifications**:
- Registration with the Ontario College of Social Workers and Social Service Workers
- 3-5 years relevant work experience
- Excellent inter-personal, communication and decision-making clinical skills
- Demonstrated ability to teach, counsel and develop written educational materials
- Demonstrated ability to work independently and as a member of a multi-disciplinary health services team
