**Clinical Care Coordinator - Temporary 6 months**
Lethbridge Family Services (LFS) is seeking a clinical care coordinator to join our team in Home Services The clinical care coordinator provides case management for clients. They work as part of a multidisciplinary team to ensure client care is delivered according to organization procedures and industry best practices.

**Clinical care coordinators**:
- co-ordinate service to each client based on authorization and service plans,
- respond to client concerns, by phone and in person,
- arrange and attend client meetings, maintain client files with current information, and documentation in accordance with organization policies and procedures,
- report unusual occurrences and conduct root cause analysis,
- conduct initial client visit and home assessment checklist and survey clients annually,
- conduct in field visits with health care aides, twice per year, and
- plan and schedule workers according to client needs, organization procedures, and funding requirements.
**Qualifications**:
- a degree or diploma in, nursing or practical nursing. Current, active registration with a professional licensing body (CLPNA, CARNA) is considered an asset,
- a minimum of 2 years of experience in care coordination or case management,
- excellent organizational and administrative skills, and
- excellent communication skills; verbal and written.
**All new employees of LFS must provide a police information check and vulnerable sector check dated not less than 90 days prior to the date of hire.**
Hours**:35 hours per week - including days, evenings, and weekends. Temporary position
- 6 months.**
