Your Opportunity:
This position contributes to the smooth day to day operations of clients having access into continuing care contracted facilities. This role is responsible for coordinating registrations, admissions and transfers to or within designated long term care and supportive Living and personal care homes based on contractual obligations. This position is critical to patient flow and the workflow is required to follow strict time lines within the contract, as well as established guidelines. This position ensures that fluid timely communication takes place between contracted sites, acute care, Integrated Home Care, Integrated Supportive and Facility living and clients. The Transition Assistants ensure that clients are registered and prioritized appropriately and in a timely manner. Accurate registration of demographics and medical information is imperative to enable appropriate transition for clients into the appropriate designated facility living option. This position supports bed management during commissioning and Bed Hub, bed matching and waitlist management as well as electronic health record corrections. The Transition Assists act as a role model for peers and office staff. They must demonstrate a high level of efficiency and professionalism at all times as they actively interact with other partners, AHS and external during potentially emotionally heighten times as client’s transition into facility care. The employee shall maintain confidentiality of all AHS information in accordance with HIA, FOIP and other privacy legislation in effect and shall comply at all times with AHS's Information Privacy and Information Security Policies.

**Required Qualifications**:
Some post-secondary education.

**Additional Required Qualifications**:
Grade 12 graduation supplemented by commercial/business courses. Medical terminology required. Excellent knowledge of the English language both written and oral. Excellent communication/organizational skills; and ability to deal with a fast paced, high volume, changing work environment. Accurate and proficient keyboarding skills 40-50 wpm required. Demonstrated proficiency of Internet Explorer and Windows based programs. Demonstrated ability to work independently. Skilled at paying close attention to detail.

**Preferred Qualifications**:
Knowledge of continuing care systems. Proficient in computer programs used by Transition Services (Pathways, electronic health records) to be able to provide IT support to Transition Services staff, AHS partners and external operators to maintain accuracy of client health record. Minimum of 3-5 years related experience in a health care or medical office setting preferred.
