**Physiotherapy ‑ Cardiac Rehab**

Under the direction of the Manager, Cardiovascular Rehabilitation and Cardiac Diagnostics, the Kinesiologist is
responsible for the planning, coordination and supervision of the daily activities of the exercise, education and
counseling programs within the Cardiovascular Rehabilitation Program. The Kinesiologist acts as the program
participant’s case manager, who develops care plans and actions to achieve increased knowledge and skills to
reduce modifiable risk factors, improving participant’s quality of life and reducing risk for future events. The
Kinesiologist will work within the interprofessional team to develop the plan of care for individuals attending the
Cardiovascular Rehabilitation Program. In addition, the Kinesiologist will participate in the provision of all aspects of
the program, including regionally based activities.
PRIMARY RESPONSIBILITIES:
- Must provide patient care or services in a manner that is consistent with TBRHSC mission, vision, values and
reflects the philosophy of patient and family centred care.
- Must treat all information concerning patient affairs or illnesses and/or hospital business as confidential, privileged
information, which must never be discussed except in the course of duty.
- Must be accountable for ensuring the safest patient care and services and for recognizing and reporting safety
incidents or potentially unsafe conditions.

at risk of cardiovascular disease.

- Design and supervise exercise sessions
- Monitor the participants response to exercise
- Provide ongoing psychosocial and motivational support in exercise sessions
- Assist in safety features of the exercise session including checking medications and compliance to prescribed
exercise program
- Assess participants for signs and symptoms of cardiovascular, or respiratory distress, and assist patient in
self‑evaluation
- Provide ongoing support for participant and family risk factor management and behaviour modification
- Participate in provision of regionally based programs via virtual care
- Construct and provide education to participants and their family in either individual or group settings, as well as,
function as an educational resource within the institution
- Construct and provide individual and group education sessions, including those provided to region via virtual care
- Facilitate and provide educational programming to peer groups within the region
- Maintain effective communication with team members
- Maintain clear and concise records regarding participant progress and care management decisions
- Participate in interprofessional team meetings regarding the planning and implementation of participant program
- Participate in clinical research
- Assist in quality management activities
- Participate in program quality improvement initiatives
- Provide clinical leadership and education to external partners
EMPLOYMENT REQUIREMENTS:
Education/Experience:
- Bachelor of Science degree in Kinesiology
- Unencumbered registration with College of Kinesiologists of Ontario
- 1‑3 years experience working in chronic disease management; specifically building and progression of care plans
Skills/Abilities:
- Minimum 1‑3 years of recent experience working chronic disease management setting
- Must demonstrate excellent interpersonal communication skills (electronic, oral and written)
- Demonstrated ability to work collaboratively in an interprofessional team environment
- Ability to clinically appraise practice related to quality improvement, ethical and clinical issues
- Ability to self‑direct activity to meet target timelines
- Demonstrated regular attendance.
- Demonstrated cultural sensitivity and awareness.
- Excellent problem solving, decision making, and critical thinking skills
- Excellent organizational and time management skills
- Experience in physical assessments
- Experience building and presenting patient and family education material
Assets:
- Successful completion of Kinesiology Graduate Diploma is an asset
- Successful completion of ACSM Exercise Specialist program
Salary Range: $35.420 to 45.130
