**Housing Support Program**

The Brain Injury Association of North Bay and Area (“BIANBA”) is a local non-profit charitable organization. We have received funding to assist individuals who are homeless, or at risk of becoming homeless, find and retain secure, safe, appropriate and affordable long-term housing. Our focus for this program is to serve individuals who identify or are identified as having a brain injury or cognitive impairment.

BIANBA makes every effort to provide barrier-free services and advocates for inclusivity and accessibility within the communities we serve. As brain injury is an invisible disability, it is often misunderstood or misidentified, which can present barriers to individuals meeting daily living needs or accessing required services. BIANBA operates with a poverty reduction lens to create opportunities for success and independence.

We are a client-centered service and staff act as “accessibility devices” to promote strategies for autonomy. BIANBA works alongside individuals to meet their needs, goals and support their choices. In every encounter, we aim to provide education/strategies to individuals and families to address the challenges of living with a complex health condition. BIANBA works with community stakeholders and service partners to advocate/educate about brain injury and its lifelong impacts.

**Position Summary**:

- functional strategies and/or access to assistive devices to address needs/challenges related to brain injury symptoms to support housing stability
- tenant education including responsibilities and rights
- collaboration with community stakeholders to support holistic wellness/wraparound support related to mental health, addictions, justice needs, education, employment, income, medical care and daily living strategies

**Specific responsibilities include**:
Timely and accurate documentation of interactions and progress is paramount.

Attend weekly Coordinated Access Nipissing (CAN) Team case conference meetings.

The use of HIFIS and the CAN process to match unique households to appropriate housing and community resources within the District of Nipissing.

Establish a caseload of eligible applicants through outreach and referrals from collaborative partner agencies.

Provide one-to-one client support and act as an advocate to develop resources and build community connections.

Engage applicants and secure permission to communicate with community service partners/professionals as required.

Conduct housing searches and build relationships with landlords in the community.

Support and accompany clients to service appointments as required.

Provide client-centered services with an understanding of income support, food security, housing, and other community social services.

Work in partnership with legal clinics, medical professionals, and other services as required.

Collect and prepare required statistical data.

Participate in meetings with project evaluation and monitoring team.

Ensure all records are kept confidential and secure.

**Qualifications**:
Education - Degree/Diploma in Social Services or other related field.

Experience - 2 years in the social services field, knowledge of acquired brain injuries is an asset.

Experience working with people who experience cognitive/intellectual impairments, mental health concerns and/or other barriers to accessing services.

Strong verbal and written communication skills.

Demonstrate an integrated anti-oppression and anti-racism approach.

Provide C.P.I.C. records check.

Working knowledge of income support in Ontario (OW and ODSP).

Ability to work independently.

Clear and strong professional boundaries.

Valid class G driver’s license and daily access to a reliable, insured vehicle required.

Current First Aid/CPR certification.

Bilingualism would be an asset.
