DUTIES:

- Assess clients at intake for eligibility and develop a community case plan.
- Provide appropriate life skills assistance, emotional support and direction as required based on a mutually agreed upon schedule.
- Connect clients with appropriate services and make any necessary referrals.
- Remain knowledgeable and up to date on community resources and information.
- Develop and maintain client files.
- Complete daily communication log.
- Advise supervisor of any critical incidents.
- Advocate on the client’s behalf.
- Other duties as may be required from time to time

QUALIFICATIONS:

- Post-secondary diploma or degree in Social Services.
- Two years’ experience working front line in a non-institutional setting.
- Strong organizational skills.
- Ability to effectively communicate with staff, families, the public and professional organizations.
- Excellent interpersonal skills and a broad based understanding of people with disabilities, addictions and mental health issues.
- Extensive knowledge of existing resources in geographically assigned areas.
- Must possess a current valid Driver’s License and have reliable transportation.

HOURS OF WORK: Maximum 28 hours per week

WAGE RANGE: $27.08-$31.65/hour
