- **KEY FUNCTION**:
Provide assessment, treatment, and intervention for older adults with mild frailty, focusing on optimizing human movement and performance in collaboration with the interprofessional team. Support professionals and caregivers with relevant resources, and help maintain a safe, therapeutic environment through active engagement with patients and team members throughout the care pathway.
** REPORTING**:
Under the general direction of the Clinical Manager, Acute and Reactivation Care Unit.
** DUTIES**:
1. Administer and perform assessments in collaboration with the interdisciplinary team.
2. Analyze and interpret assessment findings to establish patient-centered goals.
3. Gather relevant health and functional history to inform interventions, under appropriate clinical guidance.
4. Implement exercise and mobility interventions for assigned patients in collaboration with the interprofessional team, with the goal of optimizing physical function and independence.
5. Contribute to identifying frailty and restorative potential through functional mobility assessment, and collaborate with the interprofessional team to determine priority needs and appropriate exercise and mobility interventions to optimize patient independence.
6. Instruct the patient in the use of equipment and adaptive devices as directed by the the physio and occupational therapists.
7. Rehabilitate injury and the effects of disease/disability by implementing therapeutic exercise programs and other interventions.
8. Collaborate with the interprofessional team to support appropriate patient transfers from other acute care units to optimize patient flow and access to restorative care.
9. Collaborate with the interprofessional team by providing mobility assessments and recommendations that inform discharge planning.
10. Observe and document functional mobility changes that may be associated with medication effects, and communicate findings to the clinical team.
11. Educate patient’s and families in coping strategies, decision-making, and utilization of personal and community resources.
12. Collaborate with patients, families and interdisciplinary team members throughout the therapy process and advocate to ensure patient’s needs are met.
13. Document assessments, functional findings, exercise interventions, patient responses, and outcomes in the medical chart; maintain an accepted standard of documentation for quality assurance as outlined by HSN policies and the College of Kinesiologists of Ontario (COKO).
14. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
15. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
16. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
17. Educate and promote health, safety and wellness in the work place.
18. Represent the department on various committees and in meetings as required.
19. Perform other duties as required.
** QUALIFICATIONS**
**EDUCATION AND TRAINING**:
1. Minimum of a four (4) year Bachelor’s Degree in Kinesiology or Human Kinetics, from an accredited university.
2. Current Certificate of Registration in good standing with the College of Kinesiologists of Ontario (COKO).
3. Current Certificate in Basic Cardiac Life Support (BCLS) and Automated External Defibrillator (AED).
4. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
** EXPERIENCE**:
1. Minimum of two (2) years’ experience working within an acute or post acute healthcare environment.
2. Minimum of two (2) year full-time equivalent experience working in exercise interventions with patients within the last five (5) years.
3. Recent experience working with older adults is required.
** KNOWLEDGE/SKILLS/ABILITIES**:
1. Demonstrated knowledge of community support services, home and community care, rehabilitative services, adult day programs, and other social services.
3. Demonstrated knowledge of mobilizing community health resources and enhancing continuum of care.
4. Demonstrated knowledge of education curriculum development, implementation and evaluation.
5. Demonstrated knowledge of Indigenous and Francophone culture.
6. Demonstrated knowledge of Code of Ethics and Standards of Practice.
