**Position Title**: Settlement Worker - Health & Wellbeing Focus

**Reports to**: Settlement Lead / Services Manager - Settlement
**Summary**
The Settlement Worker provides direct client services and supports newcomers in integrating and adapting to life in Canada. This position has a special focus on supporting newcomer clients to build through an integrated services approach; the Settlement Worker conducts detailed needs assessment and referrals, information and orientation, service bridging, community connections, support services, resource development, and ensures service accessibility for all eligible newcomer clients. This position will deliver health and wellbeing related workshops and provide one-to-one support using a trauma-informed approach to help distressed clients and increase positive mental health outcomes for newcomers. This position provides support virtually and in person.
**Responsibilities**
- Conducts needs and assets assessments, develop service/settlement plans, and provide services bridging and referrals using client-centered and strengths-based approaches
- Designs, coordinates, and delivers group sessions and workshops on a range of health & wellbeing, mental health focus settlement topics using a variety of service delivery methods and from a gender-based analysis plus (GBA+) lens
- Assesses and responds to clients in crises or presenting with increased vulnerabilities and/or visible trauma (via phone, online, or in-person) and provides immediate support using violence/trauma-informed approaches and notifies supervisor when more in-depth service interventions are required
- Provides one-to-one sessions and group workshops to increase understanding of health & wellbeing, mental health support and to build strength and resiliency
- Assists clients in navigating the healthcare system, including connecting them to primary care providers, mental health services, and public health programs
- Offer emotional support, identify signs of distress, and refer clients to appropriate mental health professionals or crisis services
- Supports the settlement team building resilience and strength
- Keeps informed about current community, client, and service resources, trends, needs, changes, and service gaps in the general community and in specific client communities
- Conducts group information sessions and workshops using a variety of service delivery methods as suitable to meet deliverables
- Assesses and responds to clients in crises / presenting with visible trauma (via phone, online or in person) and provides immediate support and intervention, notifying others, the Leads and Managers, when a more in-depth service intervention is required
- Keeps informed of current resources, trends, needs, changes, and service gaps in the general community and in specific client communities
- Develops partnerships with service providers to effectively link clients with appropriate services support (e.g. Health facilities, Mental Health & Substance Use Services, Public Health & Preventive Services, recreation centres, etc.).
- Provides first language services as required to enhance the client experience, including providing document translation support as required
- Ensures service data is recorded in accordance with funder and organizational standards while adhering to privacy, confidentiality, and security policies
- Conducts outcomes measurement, program outreach and promotion, reporting, and client feedback collection
- Adopts performance and quality improvement (PQI) approaches in daily work to ensure the continual improvement of services to achieve program outcomes
- Complies with all organizational policies, procedures, standards, and directives, as wells as funder expectations and requirements
- Participates in organizational and team meetings, professional development and training opportunities, and evaluation and performance and quality improvement activities
- Upholds professional ethics and standards including but not limited to those pertaining to privacy, confidentiality, health and safety
- Performs other duties as assigned by Services Lead - Settlement or Services Manager - Settlement
