Greater Sudbury, Canada Continuity Care Solutions Inc. Full time

Reporting to the Clinical Manager, the Personal Support Worker will assist with patient needs under the direction of a Registered staff member on duty. Our company is called Continuity Care Solutions Inc., and this position operates out of the Alternative Level of Care Facility at Amberwood Suites, in partnership with Health Sciences North.

This position has 12 hour shifts that are variable between nights, days, and weekends. Employees usually work 60-96 hours bi-weekly.

Position Overview
- Follow site and Health Sciences North policies, processes, and procedures;
- Provide a high level of patient care by following patient care plans;
- Observe, report, and document any findings or changes in patient condition or behaviour;
- Work collaboratively within a multi-disciplinary team;
- Assist patients with ambulation when needed;
- Provide assistance moving patients in and out of facility as required;
- Other duties as assigned.

**Requirements**:

- Personal Support Worker certificate from an accredited college or equivalent;
- At least 6 months of experience as a personal support worker;
- Current Basic Cardiac Life Support Certificate / First Aid-CPR is required;
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required;
- Satisfactory Criminal Record Check with Vulnerable Sector Screening is required (dated within 6 months of employment start date);
- Updated vaccinations as per company policy and TB testing are required;
- Ability to work days, nights, and some weekends;
- Experience working with patients with dementia considered an asset;
- Experience in medication administration an asset;
- Bilingualism (French) would be considered an asset.

Pay: From $23.00 per hour
