**Job Description**:
**Education/Experience**
- Current Certificate of Competence from the College of Nurses of Ontario or Current registration with other applicable regulated health professional College of Ontario
- Baccalaureate preparation in a health-related field essential
- Master’s Degree in a Health Discipline or Health Administration preferred
- Three to five years recent and relevant management experience in an acute setting, demonstrating progressive leadership responsibilities; relevant community and/or clinic management experience preferred
- 1 year experience in the last 5 years liaising with community partners
- 2 years experience in care coordination, or advocacy and discharge planning in a healthcare setting.
- Recognized leadership/management certification preferred
- Experience with managing departmental and/or program budgets
**Competencies**
- Demonstrates strong knowledge, clinical expertise and interest in the area of geriatric care.
- Demonstrates ability to provide leadership, support and critical analysis in stressful, complex clinical and emotional situations.
- Demonstrates excellence in interpersonal, written and oral communication, including well-developed presentation skills.
- Experience in coaching, mentoring, change management, and team building skills.
- Experience in planning, coordination and implementation of quality improvement initiatives, which integrate patient and family centered care, advance best practice and support the professional development of staff.
- Demonstrates superior understanding of health systems, including Ontario Health Teams and Community based Geriatric Programs
- Possesses highly developed analytical, problem solving, & critical thinking skills.
- Demonstrates a strong patient and customer focused philosophy.
- Demonstrates superior decision-making and organizational skills.
- Demonstrates knowledge of program planning, implementation and evaluation including quality improvement methodologies.
- Ability to analyze, incorporate current research and evidence-based practice into program development and clinical practice.
- Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all.
- Commitment to providing an exceptional experience for staff and clients within a challenging and exciting health care environment.
