**You are**

A_ _**_Children's Service Coordinator_**. You provide service coordination to families and their children who meet Manitoba Possible eligibility criteria. The service coordination process utilizes a holistic, family centered approach that includes assessment, service planning, service implementation, service coordination, monitoring and evaluation. The family centred approach to service coordination emphasizes collaboration and mutual planning by the family, child, Manitoba Possible extended team and collateral systems. Service coordination is an ongoing, collaborative process in which services and resources are organized and coordinated to address the unique and changing needs of children with disabilities and their families. Manitoba Possible service coordination for children that are eligible for enrollment in the Children’s disAbility Services (CdS) Program is available. Service coordinators manage a dynamic caseload that ranges in size, includes ongoing intake as assigned by the program manager including children ranging in age from birth to school leaving age.

This is a.5 EFT, 15-month term position.

**Your Role**
- Develops, implements, monitors and evaluates comprehensive service plans in partnership with family members that delineate needs and goals of children and their families. Maintains a working knowledge of policies and procedures and current best practices that meet professional standards and complement an integrated service delivery system.
- Provides the program manager with timely feedback including statistical reports, quarterly reports, ongoing perspective on outcome achievement and identification of service gaps and individual supervision needs.
- Establish and maintain a collaborative relationship with the child, their family and significant others in order to maximize the client’s participation in the assessment of needs and the development of plans.
- Review and revise the service plan through individual or team meetings to ensure relevant goal setting and goal completion.
- Provide information and practical help to the clients, family, and significant others and promote self-leadership skills.
- Monitor relationships, plans, and progress to ensure timely service implementation and ongoing proactive planning, including the CdS Service Plan Request process and annual projections tracking process.
- Assess and promote the abilities and potential of clients, families, and significant others.
- Ensure current understanding of the client’s changing abilities through facilitating comprehensive assessments.
- Identify systemic issues that prevent full participation and inclusion.
- Provide referral, consultation, and information to facilitate client access, coordination and utilization of available community and Manitoba Possible resources.
- Participate in regular supervision.
- Participate in relevant team meetings, special projects, committees and in-service training related to the responsibilities inherent in the job.
- Carry out and maintain all requirements inherent in the Manitoba Possible case management system, CdS policies, and regional procedures.
- Maintain collaborative relationships with community resources, handle inquiries about agency services, ensure that Manitoba Possible services and programs are known, and obtain feedback from the community on Manitoba Possible services.
- Participate in the hiring and supervision of staff and students.
- Maintain professional integrity including acting in a professional manner, representing Manitoba Possible interests in daily activities, self-initiative in problem solving and exercising sound judgment, working within deadlines and upgrading skills in accordance with changing methods.

**What You Know**
- Bachelor of Social Work with 3-5 years of service coordination/case management experience.
- Knowledge and understanding of human behaviour, growth, and development.
- Knowledge of neurological and/or physical disabilities and their impacts on children and families.
- Knowledge of Independent Living Paradigm, Inclusion Philosophy and Community Development.
- Knowledge of a family centered model of service delivery, child development, family dynamics, and community resources.
- Skills in assessment, individual/family counseling, inter-disciplinary teamwork and group work.
- Strong communication skills, both oral and written.
- Demonstrated basic knowledge and experience with computers.
- Driver’s license and access to a vehicle required.
- Knowledge of Deaf Culture and American Sign Language (ASL) an asset.
