Brantford, Canada St. Joseph's Villa - Behavioural Support Ontario (BSO) Full time

1. Works collaboratively with care partners and consultants serving long term care homes. This includes HNHB Placement Services, Alzheimer Society PRCs (Psychogeriatric Resource Consultants, Specialized Geriatric Services, Community BSO services, LTCH BSO Mobile, BSO Transitional Leads and discharge planners and other health service provided as required.

2. Completes required/ identified assessments and assists in the development, modelling and coaching of care strategies based on the referral received and the need identified by the referring home or team member.

3. Works with the specific LTCH leadership team to support the needs of the individual and reports on the care plan, assessment and findings to the same.

4. Participates in Continuous Quality Improvement practices as identified by the BSO Program, its Director or Oversight Committee through data collection, analysis, improvement initiatives and PDSA cycles as directed/ requested.

5. Supports the resident and their caregivers by providing counselling and supportive care to residents.

6. Provide recommendations on bio-psychosocial and non-pharmacological interventions to support resident goals in collaboration with the resident/ their family / significant other and the interdisciplinary care team.

7. Uses approved PIECES and other relevant assessments as outlined by the program.

8. Works within assigned hours and locations as agreed upon with the Sub Region manager in a case based manner.

9. Ensures care strategies progress and are regularly evaluated and adapted in consultation with the LTCH care team and BSO scheduled staff.

10. Provides mentorship and support that is person-centred and based on best practices.

11. Identifies themes and opportunities for educational in-services in collaboration with the LTCHs, other service providers, outreach services/teams and experts in the field.

12. Utilizes therapeutic communication by establishing a therapeutic relationship with clients, families, co-workers and others for the purpose of meeting client needs.

13. Collaborates with LTCH staff/leadership, families, and service providers in a professional, courteous manner and keeps the BSO Sub Region Manager and/ Behavioural Lead/ Clinical Coach informed of any changes or concerns identified.

14. Provides support to the care team by modelling strategies to mitigate escalating responsive behaviours to LTCH staff.

15. Participates in ongoing education and training related to best practices and responsive behaviours to the target population.

16. Participates in all BSO sub-regional staff meetings, education and/or training as required by the host employer St. Joseph’s Villa.

17. As a team member, supports successful resident care transitions by facilitating care plan recommendations identified by the BSO Transitional, hospital Clinical Leader, community partners and BSO family of services in shared clients/ residents.

18. Documents visit notes and assessments according to BSO standards, within the residents chart and according to College of Social Work Standards.

19. Identify opportunities to build capacity of LTCH staff through mentorship and coaching.

20. Carry out all duties with appropriate attention for resident/client safety.

21. Responds in Emergency situations such as bomb threat, fire, Code Yellow, Code White as per established procedure in the LTC home and only at the request of the LTC home as a guest.

22. Complete other duties as assigned by the BSO Mobile RN Manager.

**FORMAL EDUCATION**
- Degree in Social Work (Registered, BA or Masters) in good standing and must maintain College Registration at all times.

**OTHER KNOWLEDGE, SKILL AND EXPERIENCE**
- At least 2 years’ experience; Long Term Care, specifically working within a Mental Health setting or setting providing services to older adults with responsive behaviours related to dementia, mental health or other factors.
- Training in Montessori, GPA, adult mental health is an asset
- Demonstrated leadership and ability in coaching, teaching, mentoring staff
- Ability to adapt positively and respectfully to changes in work environment and work location
- Valid Drivers’ License
