**Job Description**:
Reporting to the Manager, Patient Safety and Patient Experience, the **Patient Safety Consultant** supports the organization’s frameworks; Quality and Safety, infrastructure, protocols, analysis methodology and reporting process to identify, analyze, quantify and monitor quality and patient safety across all domains with the key objectives of quality improvement, patient safety, risk avoidance, loss prevention and maximizing stakeholder value.

**Education**:

- Undergraduate Degree in Health Sciences, Leadership, or Quality Management - Mandatory
- Masters in Health Sciences, Leadership, or Quality Management - An asset
- Clinical designation - Mandatory
- PMP, Six Sigma, LEAN Certification - An asset
- Change Management Foundations and Practitioner - An asset
- Certified Professional in Patient Safety - Preferred

**Experience**:

- Minimum of three to five years performing root cause analysis for patient safety events and developing meaningful and actionable recommendations that will prevent harm and promote patient safety
- Minimum 3-5 years professional experience in a similar role, preferably in a complex healthcare setting

**Competencies**:

- Working knowledge of current legislation of Governing Health Care Facilities (e.g. Excellent Care for All Act, Public Hospital Act, Mental Health Act, Quality of Care Information Protection Act, and the Coroners’ Act)
- Working knowledge of regulatory bodies
- Extensive knowledge of Canadian Quality & Patient Safety Framework
- Working knowledge of patient incident reporting systems including consistent incident follow up system
- Demonstrated knowledge of adult learning styles and advanced consulting methods
- Facilitating instructional skills (MBTI, etc.) and/or Coaching Certification (Accredited program) a strong asset
- Previous work experience with metric-based performance evaluation systems (i.e. Balanced Scorecard)
- Demonstrated ability to plan and facilitate
- Strong knowledge of measurement and goal setting methodology
- Ability to engage key contributors in a timely and sensitive manner, align and deliver outcomes within established timeframes
- Proven ability to develop relationships and influence others collaboratively to resolve difficult situations
- Rapid assessment and critical thinking
- Strong presentation and communication skills with a strong ability to coach, mentor and motivate others
- Results oriented self-starter with the ability to manage multiple, competing priorities in a fast-paced environment
- Ability to draw insightful impressions from objective data to identify process and organizational culture issues in need of improvement
- Superior analytical, problem solving, research, relationship building and conflict resolution skills
- Navigate/strong knowledge of Electronic Medical Record (EMR) software such as Meditech Expanse
- Knowledge of the Ontario healthcare system, applicable legislation, administrative, clinical and departmental principles and protocols
- Understanding of cultural dynamics and funding structures in a health care environment

**Responsibilities**:

- Lead Patient Safety Incident Reviews utilizing gap analysis and work to promote quality improvement
- Provide support and coaching on Always Practices
- Monitor Quality of Care Review and Incident Review recommendations and support leaders to ensure quality improvement is obtainable in a timely manner
- Ensure quality and safety activities are in compliance with relevant legislation and corresponding regulations and accreditation requirements
- Work collaboratively with key stakeholders on the implementation of the organization's Quality Plan
- Analyze data and information to successfully formulate strategies to ensure regulatory readiness and quality outcomes
- Provide onsite support to front-line staff to ensure quality and safety guidelines are being met in accordance to hospital standards
- Consult with and provide advice to organizational leaders regarding error prevention strategies; design programs that employ best practices to reduce preventable adverse events across the organization
- Review incident reports and investigate actual/potential, safety/liability issues
- Take a lead role in strengthening a culture of patient safety which encourages prevention, reduces errors and safeguards patients from harm
