**Nature & Scope**
Reporting to the SVP, Health Equity, Child Care & Camps, the Director will work collaboratively with YMCA BC chronic disease program staff and key stakeholders. The Director is responsible for the development, implementation, and evaluation of chronic disease programming across the regions that YMCA BC serves. The Director will learn from our current best practices of chronic disease program delivery and where possible will work to implement these pockets of excellence across our regions. In addition, the Director will source out new program opportunities that positively impact health and well-being, from a health equity lens. The Director is responsible for ensuring the best practice implementation of our chronic disease programming that improves health and wellness outcomes while ensuring financial viability of these programs.
The Director is responsible for the strategic planning of this portfolio, oversight of all chronic disease operations, development of new chronic disease programs, quality of practice, risk management, and financial management.
**Major Responsibilities**-**_in this position, you will_**:
- Work with leaders and staff of chronic disease programs across YMCA BC to ensure the delivery of high-quality programs and services that improve the health and wellbeing of program participants while meeting the standards of funding agreements.
- Develops and maintains positive relationships with stakeholders and potential funders, pursuing additional chronic disease funding opportunities
- Develops new chronic disease programming that improves the health and wellbeing of British Columbians, from a health equity lens.
- Promoting the YMCA is a charity internally and externally that is dedicated to health equity
- Ensures positive working relationships with chronic disease teams, inclusive of Community Programs leadership and Health, Fitness and Aquatics leadership and staff that are responsible for chronic disease implementation.
- Ensures financial viability of chronic disease programming through financial planning and administration of operating budgets, and analysis of fee/ membership funded programming
- Supports the development of program evaluation practices by collaborating with internal leadership and partners to collect data in a timely, efficient, and effective manner. Complete reports as required and oversee the ongoing monitoring and evaluation of programs to ensure continuous improvement
- Has responsibility for ensuring the safety of all program participants and staff who participate in chronic disease programming.
- Supports chronic disease program leadership to promote enrollment of all chronic disease programs, including marketing, promotions and outreach.
- Develop materials for the Health Equity Task Force, and other Executive Leadership requests.
**Qualification**-**_you bring_**:
- Post secondary degree in a health-related discipline such as Health Science, Public Health, Kinesiology, Human Kinetics, or other related field
- 7+ years of experience in Chronic Disease Programs or equivalent combination of education and experience
- Program coordinator or management experience, including supervisory experience
- Experience in the development and or expansion of programs
- Knowledge of chronic health conditions including heart disease, diabetes, hypertension, stroke, etc.
- Experience in financial management, including developing and monitoring budgets
- Experience with oversight, developing and or facilitating health and wellness programming
- Experience in community outreach and building relationships with diverse stakeholders
- Exceptional interpersonal, communication and conflict resolution skills
- Computer proficient with proven experience with Office 365
