**Position Summary**:
The Clinical Manager will provide leadership to a team of clinicians with specialty knowledge and expertise in chronic disease, serious illness, palliative and end of life care. Additionally, to support care in these areas of speciality practice, the Manager will lead provincial consultation, practice support and education for the home and community care programs, palliative and end of life care in First Nations communities.
The Manager is responsible for leading the collaborative strategy development and support to FNHA Regional teams and First Nations Communities for clinical services for clients with chronic disease, serious illness and who are receiving home and community care services and/or palliative or end of life care. Working in partnership with regional and provincial partners, the Manager will lead with a focus on culturally safe and innovative inter-professional team based quality care. The role will support the ongoing design, planning, implementation, and evaluation of services that enable innovative, evidenced based, best practice care.
**As a health and wellness organization, the First Nations Health Authority has aligned with the Provincial Public Health Order released on October 14, 2021. We have a mandatory COVID-19 vaccination policy for all employees at First Nations Health Authority. This policy requires all employees to be fully vaccinated against COVID-19. **Responsibilities**:
- Leads and models cultural safety, humility, relational practice and a trauma informed approach in all interactions, partnerships and planning initiatives.
- Leads and manages an interprofessional team of clinicians who have specialized knowledge and expertise in chronic disease, serious illness, home and community care, palliative and end of life care.
- Leads and promotes clinical change initiatives, new knowledge and innovation and knowledge translation activities that enhance delivery of services. Supports the implementation of best-practices and evidence-based standards as it applies to the continuum of community
- based services.
- Works in collaboration with, and provides advisory and consultative services for home and community care services to, First Nations communities and partners, health organizations, community members, nursing professionals, social workers, and caregivers Holds accountability to manage the operational requirements of the team and program including HHR planning and processes, strategic recruitment and retention, OHS, budget, reporting/surveillance and other related accountabilities.

**Qualifications**:
- A Master’s Degree in a relevant field, or an equivalent combination of education, training and experience
- Relevant professional development in areas of specialty clinical practice, leadership, process improvement, change management.
- Recent, relevant experience, of at least 8 years, at a senior level working within a complex healthcare setting with a focus on Chronic Disease, Serious Illness, Home and Community Care, Palliative and End of Life Care.
**Knowledge, Skills & Attributes**:
- Demonstrated knowledge and ability in leadership, strategic and systems level planning, clinical health care practice, program planning, development, implementation and evaluation.
- Awareness - Thorough knowledge of the current First Nations health governance landscape in BC.
