**Position Overview**

We are seeking a compassionate and skilled **Intensive Case Manager (ICM)** to provide temporary coverage until March 31, 2026, with the possibility of extending this role until March 31, 2027. This role is essential in helping participants find and maintain suitable housing while achieving an optimum quality of life through comprehensive support services.

**Key Responsibilities**
- Complete intake assessments for each participant received via referral
- Develop person-centered case plans with tangible housing stability goals, timelines, and clear accountabilities
- Conduct risk assessments and create crisis response plans for each individual
- Provide excellent organizational skills in database and file management, including detailed case file reporting
- Assist participants in identifying and overcoming barriers to housing stability
- Locate and access appropriate, affordable housing based on participant preferences
- Support participants through the lease signing process and moving procedures
- Provide direct assistance with furnishing apartments and ensuring basic needs are met
- Interact professionally with landlords on tenancy matters
- Re-house participants when necessary while maintaining continuous services and supports
- Facilitate move-in and move-out processes, including unit cleaning
- Serve as an effective mediator and advocate for all housing participants
- Broker and advocate for access to mainstream services including physical health, mental health, addiction services, income supports, and recreational activities
- Organize and promote meaningful daily activities for service participants
- Teach and model independence skills using adult learning strategies, including: setting up and attending appointments, rent payment and household management, etc.

Required Qualifications
- **Education**: Undergraduate degree in relevant field (Social Work, Psychology, Public Health, Addictions Counselling, etc.) or comparable experience
- **Experience**: Working with similar populations including individuals who are homeless, have mental illness, chronic health conditions, addiction disorders, or developmental disabilities with complex needs
- Experience working with high-risk populations
- Knowledge and experience in harm reduction strategies, crisis intervention, and therapeutic communication
- **Certifications**: First Aid and CPR
