**What we are looking for**:
The BRIDGE Day Treatment Program is a group-based weekday program within the Mental Health System, Nanji Mental Health Services. You will be part of a multi-disciplinary team working with patients to support an alternative to hospitalization. Part of your role with outpatients will be that of a care coordinator to assist in developing patient goals, monitoring their progress, and planning discharge.
The responsibilities of this role will include the following:
- Planning, developing and evaluating group programs
- Leading/Co-Leading group programs
- Coordinating patient care
- Participating in interdisciplinary clinical rounds/meetings
- Connecting with Community resources
- Participating in Quality management initiatives
- Participation in Quality Based Procedures for Schizophrenia training and support for individuals and families
**What you bring to the role**:
- Masters of Social Work degree (MSW)
- Current registration with Ontario College Social Work and Social Service Workers (OCSWSSW)
- A minimum of 5-7 years of previous experience with assessment and treatment in adult mental health setting
- Clinical experience in working with adults who suffer with mental health diagnosis and practicing from a trauma informed perspective
- Demonstrated clinical experience in leading therapy groups; Cognitive Behaviour Therapy (CBT), Dialectical Behaviour Therapy (DBT), IPT, Interpersonal Therapy for Depression (IPT), Mindfulness
- In person treatment skills as well as comfort and skill providing patient care, both 1:1 and group, with virtual technology such as zoom
- Excellent interpersonal, oral and written communication skills
- Previous experience with program planning, development, and evaluation of group and/or individual therapy
- Proven effective collaborative skills as a member of a inter professional team
- Proficient computer skills including Microsoft Word, Excel and Power-point
- Demonstrated commitment to professional development
- Demonstrated good attendance and performance records with the ability to maintain these same standards
- Demonstrated commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital related policies, health and safety legislation and best practices and completing relevant mandatory education as required
**Work Schedule**:Monday through Friday, daytime shift, generally 8:30-4:30. Occasional later day work or rotating Saturdays if needed
**Compensation**:
Band I $39.46-50.27
