**Lead Case Manager**
**Purpose of Position**
- The Lead Case Manager monitors the implementation of individualized learning programs and supports LDS Instructors with best practices. The position reports to the Director of Education and works collaboratively with LDS staff to ensure the efficacy of service delivery.**Roles and Responsibilities**- Lead LDS Case Management team and LDS’s one-to-one remedial education programs-
- Facilitate communication between Instructors and families and provide solutions as appropriate-
- Support the Instructional team in administering informal, formal, and standardized learning assessments to evaluate student strengths and learning needs on an ongoing basis-
- Analyze and monitor data from standardized assessments-
- Coordinate and/or write student debriefs to ensure individual learning goals and strategies align with clinical and/or assessment data-
- Participate in student placement meetings with the student and family services intake team-
- Support Case Managers and Instructors with identifying and developing learning objectives for the school year-
- Evaluate individualized learning programs to ensure they reflect the most relevant information, such as the student’s learning needs, assessment results, debrief information, progress monitoring data, and long-term learning goals-
- Conduct scheduled session observations (informal and formal) to provide feedback on instruction and determine the need for additional reinforcements, resources and/or adjustments to instruction. Provide additional observation support as needed or by request.-
- Support in the screening and hiring of new Instructors-
- Proactively welcome and provide onboarding support for new Instructors by setting up an introductory meeting to review Case Management role in supporting instructors-
- Delegate tasks and/or ask for support as needed to maintain the integrity and quality of Case Management services-
- Provide administrative support as appropriate**Qualities**- Ability to work independently-

- Passionate about inclusion**Knowledge and Skills Required**5+ years of relevant professional experience
-Bachelor’s Degree in Education or a related field (minimum)
-Graduate studies or certificate in Education/Special Education or a related field (preferred)
-Experience working with multiple disabilities/differences, including learning disabilities, ASD, ADHD and developmental delays (preferred)
-Criminal record check with the vulnerable sector
-Eligibility to work in Canada
**Position Details**-
-Competitive vacation and benefits package
-Salary range: $58,000-$62,000 per year dependent on education and experience
-Location: Primarily based in our East Vancouver Learning Centre, however, flexibility to also work out of our North Vancouver Learning Centre on occasion is required.
