**Key Responsibilities**:
- Coordinate new referrals from intake through to service initiation
- Connect with clients to gather key information and provide clear, supportive explanations of the process
- Document client needs related to physical, emotional, and cognitive challenges
- Prepare and manage intake forms and digital documents, ensuring timely completion and follow-up
- Maintain ongoing communication with clients and referral partners to provide status updates
- Track the progress of each intake to ensure nothing falls through the cracks
- Build strong relationships with legal, healthcare, and community partners to support seamless client care
- Provide thoughtful, trauma-informed communication to individuals who may be overwhelmed or in distress
- Support clients in understanding next steps and available services
- Contribute to a high-quality, client-centered experience from first contact to service launch
