**JOB SUMMARY:
The Waterloo Wellington Integrated Stroke Program (WWISP) leads all aspects of stroke care within Waterloo Wellington (WW) hospitals through an integrated system of care, spanning prevention, pre-hospital care, acute care, and community and hospital based rehabilitation. Grand River Hospital (GRH) is the lead organization for Stroke care within Waterloo Wellington (WW). Acute Stroke care is provided at GRH and Guelph General Hospital (GGH) and in-patient rehabilitative care is provided within GRH, St. Joseph’s Health Care Guelph (SJHCG), and Cambridge Memorial Hospital (CMH). Understanding out-patient, in-home and community based rehabilitative care for stroke patients are also important parts of the continuum of stroke care.
The WW Stroke Navigator will be accountable to the Director of the Emergency, Medicine and Regional Stroke Program for the appropriate referral and placement of stroke patients to ensure capacity and flow across the continuum of the system within all organizations. The Navigator will work as an integrated member of the health care team and in conjunction with partners within WW and the community will consult, educate and make recommendations for patient care and transition planning. Contributes to ensuring there is a safe environment for patients, staff and visitors.
The Stroke Navigator’s main areas of responsibility include:
1) Review and triage of patients in the stroke system
2) Clinical consultation to ensure the right client receives the right service at the right time through management of wait lists, coordination of admissions and discharges, provision of interim monitoring, consultations and crisis management
3) Clinical Monitoring, Advocacy and Service Development and Evaluation
4) Case Management
**QUALIFICATIONS:
- Baccalaureate degree in a recognized health care discipline
- Regulated Health Professional in good standing with their regulatory College with a minimum of 5 years in a clinical setting.
- Knowledge of/recent clinical experience in WW health care system preferred
- Recent experience (within the past 2 years) in acute stroke and/or stroke rehab preferred
- Able to work independently - self directed
- Able to discern relevant clinical and system information under pressure to make appropriate decisions regarding patient care planning
- Demonstrated knowledge of quality Improvement and change management process
- Demonstrated knowledge of community stroke resources
- Highly motivated team player with excellent organizational, leadership, problem-solving, collaboration, decision-making and communication skills
- Possess an understanding of the components of care within the continuum of stroke care and of best practices related to transitions of care
- Client/patient service oriented, able to work effectively with diverse populations/groups
- Demonstrates excellent skills in consultation, negotiation, mediation and client advocacy
- Familiar with data collection, evaluation and dissemination
- Able to travel within the LHIN as required with own vehicle
