**Job Summary**
This position involves providing support to 39 residents with Complex Needs, and will have a caseload of 10-11 McEwen Manor residents. In this position, you will be working under the direction of the McEwen Manor and CLSD Program Manager with the goal of providing case management and everyday support services. This can include but is not limited to things such as taking residents to appointments, shopping, meal preparation, social/recreational activities, facilitating educational groups, and assisting with budgeting, monthly reporting, conducting assessments and compiling annual Wellness Plans.

**Duties**:

- Implement psychosocial rehabilitation plans to assist individuals in achieving their recovery goals
- Conduct assessments and develop individualized care plans
- Provide counseling and support services to clients
- Collaborate with healthcare professionals to ensure comprehensive care
- Maintain accurate and detailed client records
- Monitor progress and adjust rehabilitation plans as needed

**Qualifications**
- Degree in a related field (Social Work, Psychology, Human Justice)
- First Aid/CPR
