**Position Summary**:
Our Practice Consultant, Elder Care provides professional practice, education, and clinical practice consultation support for health care providers, providing care and service for First Nations Elders in community. In this position, you will contribute to program planning, guideline and resource development to support knowledge/practice in areas of elder health, including health diagnoses, home care, wounds, continence, foot care, chronic conditions, dementia, falls, and elder abuse. **Responsibilities**:
**The responsibilities of this position include**:
1. Works in collaboration with FNHA partners, regional teams, community health care providers and community members, Elder and Knowledge Keepers to plan relevant and culturally safe programs, services, and resources for Elders.
2. Plans, organizes, and delivers a comprehensive range of professional services, including consultation, assessment, facilitation, teaching, and evaluation to further the development of knowledge and skills for nurses and other health care providers caring for Elders and others receiving home care services.
3. Coordinates, plans, arranges, and delivers courses, workshops, and other educational sessions to meet the needs and priorities of nurses/health care providers in communities and centralized locations. This may also include wound, ostomy, foot care or continence support.
4. Collaborates with provincial and national agencies/ specialty societies that support Elder care and services. Contributes specialty knowledge to the development of clinical guidelines, policies and resources supporting Elder care and home care.
5. Provides / contributes knowledge of Elder program/services for community development and investment decisions (eg: HCC program services and goals, budget surplus investment decisions).
6. Utilizes a quality improvement approach in program care and planning and contributes to quality improvement activities by participating in groups, teaching, analyzing quality improvement data, and making recommendations regarding areas of improvement.
7. Provides data to assist in the measurement and analysis of performance indicators/client outcomes and in production of reports.

**Qualifications**:
**To be successful in this role you will need**:
**Education**
- Bachelor’s Degree in Nursing
- Advanced education/certification in gerontology is preferred
- Wound Care Course is preferred
- Adult Education Certificate is preferred

**Occupational Certification**
- Current practicing registration with the BC College of Nurses and Midwives

**Experience**
- Five (5) years of experience in clinical practice caring for Elders, including experience working in home and community care
- Experience in planning and delivering educational programs for nurses/health care providers

**Knowledge, Skills & Attributes**:

- Knowledge of, and ability to apply, an understanding of First Nations cultural principles and protocols in work situations.
- Knowledge of traditional, alternative, complementary, and other recognized healing practices.
- Knowledge of concepts, principles and theories of trans-cultural nursing, including knowledge of traditional, homeopathic, or other healing practices used in the community.
