As a Community Health Manager (Sales Territory Representative), you will play a pivotal role in establishing and nurturing relationships within the community. This position requires proactive engagement with potential clients and referral partners, including healthcare professionals and facilities, to promote our services and foster collaboration. You will primarily be responsible for face-to-face marketing, attending community events, and conducting in-home assessments.

**Responsibilities**:

- Proactively build and maintain relationships with potential referral partners, including social workers, hospital staff, and care facilities, Doctors, Pharmacists, Denturists & Occupational Therapists etc.
- Educate potential partners and clients about our services, demonstrating how we can meet their needs and improve quality of life.
- Develop and execute marketing strategies within the healthcare field and community events to increase brand awareness and generate leads.
- Conduct in-home assessments, create personalized care plans, and effectively close leads and opportunities.
- Utilize MONDAY and other systems for lead management and follow-up activities.

**Key Requirements**:

- Excellent communication skills, both verbal and written.
- Sales and marketing experience, preferably in the healthcare industry.
- Healthcare experience is an asset, with a willingness to learn about our industry and clients' needs.
- Proficiency in using various computer systems for documentation and communication.
- Active listening skills and a genuine passion for delivering quality care.
- Valid driver's license and access to own vehicle (non-negotiable).
