The Graduated Rent Subsidy Program (GRSP) works with individuals and families who have successfully completed a Housing-First Program within Calgary’s Homeless-Serving Sector of Care (HSSC). The GRSP supports those who require a rental subsidy to maintain housing stability, but who no longer require Intensive Case Management support. The program aims to improve economic, social and community supports and foster skills to achieve independence and graduation.
The Housing Coordinator, Subsidy & Graduation is responsible for providing supports to clients to assist them in maintaining housing stability after the client has graduated from a Housing First program. The Housing Coordinator will provide community-focused strategies, resources and supports to expand on the success the client achieved in their previous Housing program. The Housing Coordinator will work with the client to ensure the financial, health and education goals of the client can be met by linking client with appropriate supports both internally within CUPS and with external programs. The Housing Coordinator will assist clients in their goal planning, provide information, resources and system navigation to support achievement of graduation.
**Leadership**:
- Create approachable, caring and trauma informed environment for clients, guests and colleagues
- Provide person-centred services based on the clients’ strengths as well as complexities of need
- Build strong relationships based on respectful engagement, openness, listening skills, non-judgmental attitudes and advocacy
- Ensure confidential client information is handled in alignment with CUPS privacy protocols
- Operate in synergy with CUPS internal departments continuously assessing client needs, gaps in service, care plan modifications and advocating for the voice and self-determination of the client
- Facilitate a healthy and safe work environment through learning and enacting all CUPS health & safety protocols, while maintaining an active awareness of clients and colleagues
- Develop and maintain strong community partnerships with community and government services
- Promote a culture that reflects CUPS Purpose, Ambition and Values
**Organizational Alignment and Performance**:
- In collaboration with the client, develop strengths-based graduation goals using CUPS Integrated Care Tool to review goal progress with clients
- Within a designated caseload develop strong working relationship with clients to identify goals, connecting clients to community and CUPS supports to assist in the achievement of graduation
- Provide culturally informed support to CUPS clients
- Screen and assess housing stability, supporting housing interventions to support tenancy
- Provide effective communication related to subsidies impacting tenancy and complete income testing to promote housing stability
- Perform duties alone and with colleagues in a variety of settings, including meeting clients at CUPS as well as at external appointments, service providers and agencies, and community-based meetings related to social, recreational, financial and vocational supports.
- Accurately enter all required data points and required consents
- Remain up to date on changes to programs, services and personnel to ensure accurate care plan development and effective referrals
- Develop and maintain extensive knowledge of community social supports and services available to connect clients to services supportive to program graduation
**Stakeholder Relations**:
- Develop and maintain collaborative and accountable working relations with internal CUPS colleagues to strengthen communication flow and cross-program understanding
- Foster excellent relationships with and facilitate case coordination with all client service providers & housing locator to reduce gaps in services
- Provide caring, reliable, trauma-informed care to all clients accessing CUPS to reduce barriers to service and increase community trust
- Share experience and knowledge with students, volunteers and guests interested in CUPS services to further develop CUPS social justice impact & ext
