**St. Leonard's Place Peel is a community based residential program that provides a place of safety, belonging, dignity, nourishment and education for homeless men with serious mental illness, addiction and those who have been in conflict with the law.**
The Intake and Outreach Worker is a dynamic role that will support cross-functional duties across the agency’s Intake and client services portfolios. The primary goal of the position is to connect prospective clients in the general community that are experiencing chronic homelessness to housing support and outreach services while on the waitlist to receiving housing; as well as to offer specialized supports to transient men at risk of experiencing homelessness.
The intake component of the role will provide information, referrals and a “fit” assessment to clients in the community seeking access to region-wide services. This role will also provide outreach to clients in the community to provide access to SLPP’s resources and provide specialized care plans. This role will also support men in St. Leonard’s Place Peel Stay The Night program and provide brief community support.

**General Intake and Correspondence**
- Provide information, referrals and system navigation to clients seeking information about SLPP services
- Complete in person assessments for clients who qualify for the program
- Support the coordination of external referrals.
- Develop and maintain community partnerships with organizations.
**Outreach/Assessment Support**
- Conduct travel to designated community locations to enhance outreach efforts.
- Meet with clients to effectively communicate the program and available services.
- Collaborate with partner agencies to optimize resource sharing and service delivery.
- Ensure timely follow-up on referrals within two business days.
- Disseminate comprehensive information regarding SLPP’s services to various stakeholders.
- Facilitate access to immediate needs identified during the intake process.
- Maintain regular daily follow-up with clients to support their ongoing needs.
- Co-develop personalized housing plans tailored to individual client requirements.
- Provide intermittent supportive counseling as needed to assist clients.
- Develop and maintain detailed case notes documenting client progress and support provided.
- Engage clients in meetings promptly upon receiving referrals.
- Assist clients in their orientation to stable housing upon intake.
- Ensure access to hygiene products and nutritious meals for clients.
- Support clients through the preparation process and aid in housing retention efforts.
- Correspond with community partners and landlords on behalf of clients to facilitate housing solutions.
- Collaborate with the Intake and Reintegration Worker to identify and utilize relevant community resources.
- Collaborate with partners to refer individuals to the By Name list.
- Provide support and assistance in the community to men of the Stay The Night program.
**Administration and Case Management**
- Implement evaluation tools to measure program efficacy and growth over time.
- Maintain individual client files and compile statistical reports.
- Complete Incident Reports as necessary.
- Prepare daily logs and notes in accordance with agency policies and procedures as required.
- Provide client monitoring and accommodations in alignment with the program’s mandate.
- Generate weekly statistics for the Region of Peel and CEO reports.
- Create and regularly update client files.
- Distribute satisfaction surveys to all clients to gather feedback.
**Aftercare Support**
- Arrange follow-up meetings or check-ins with clients to evaluate progress, tackle challenges, and offer continual support.
- Offer educational resources and advocacy assistance to clients, empowering them to navigate systems and access required services independently (part of the transitioning process involves reducing dependency on the support provider).
- Keep precise records of client engagements, advancements, and results, documenting aftercare strategies and follow-up initiatives.
- Work together with other service providers, agencies, and community organizations to guarantee ongoing care and support for clients after discharge.
**Requirements**:
- Bachelors of Social Work (BSW), or diploma in Community, Social Services, Criminal Justice with a combination in education and experience.
- Congregate living experience is an asset
- Pass security clearance checks.
- Minimum 3 years’ experience working with men with mental health, justice and housing challenges.
- Have advanced knowledge of homelessness.
- Have a background in crisis intervention.
- Ability to travel to other organizations when needed.
- Valid Drivers licence and access to a reliable vehicle
- Excellent skills in communicating effectively, both orally and in writing.
