**The Role**:
Are you a patient focused Lab Technician with medical administration experience and excellent written and telephone communication skills?
KMH Cardiology Centres Inc. is growing and we are recruiting for a Patient Care Coordinator (PCC) for our busy practices in **Toronto **and **North York**.
The PCC is responsible for providing empathetic and efficient indirect care for patients attending KMH labs.
Scope of role
- under physician direction the PCC:
- Provides guidance, support, education and advice to patients
- Serves as a liaison between members of the multi-disciplinary team and external service providers to ensure:
- High standards of quality and optimal management of patient care outcomes
- Ongoing continuous improvement practices to support clinic processes
**The Accountabilities**:
- Coordination of daily clinic operations
- Patient Intake
- Management of Physician orders
- Tasks Management
- Respond to patient enquiries regarding clinical care within 24 business hours
- Manage missed appointments and demonstrate ability to empathize with the complexity and difficulty of each patient when following up on missed appointments
- Inform and educate patients about tests and procedures as requested by physician
- Process prescription renewal requests as directed by physician in collaboration with administrative staff within 24 business hours
- Foster a collegial, respectful clinic environment
**Minimum qualifications**:
- Lab Technician (certified)
- Medical administration experience
**Required skills and experience**:
- Proven ability to work proficiently in a busy fast paced specialist clinic environment
- Self-directed, detail oriented with an ability to multitask, establish priorities and achieve objectives with mínimal supervision
- CPR
- Strong knowledge of medical terminology
- Excellent verbal and written communication skills
- Superior telephone etiquette
- Strong conflict resolution skills
- Proficiency in technology including MS Office and EMR systems experience
- Excellent time management and organizational skills
- Possesses effective analytical and problem solving skills
- Able to deal with sensitive information with a high degree of diplomacy
- Team orientation
- The ability to provide coverage at two Labs: KMH Toronto and KMH North York

The Patient Care Coordinator team takes pride in our patient focused environment and works very closely with other service and clinical teams at KMH.
This is a full-time role scheduled for 5 days per week (between Monday-Saturday) during the hours of 8.00 a.m.4.00 p.m.
The role is based at our **Toronto **and **North York** labs. This position reports to the Team Lead: Patient Care Coordinators.

**Salary**: $20.00-$22.00 per hour
