Using the “living classroom model”, the Practicum Educator provides evidenced based education using appropriate methods, including teaching, demonstrating, lecturing and peer mentorship in compliance with Bayshore policies & procedures, and CHW curriculum.

ROLE
To provide oversight and support to CHW students throughout the CHW Program. This will include preparing the practicum experience and providing the hands-on Skills Training in preparation for their practicum experience. The Practicum Educator will also provide ongoing support, supervision and evaluation of the practicum.
Specifically, the Practicum Educator will also ensure all CHWs are identified in the various databases (PeopleSoft, Procura etc), will ensure CHWs have login credentials for access at time of hire, will support any issues identified during theory, will facilitate weekly team meetings and orientation for the CHW and to assign preceptors and manage the schedule.
The Practicum Educator will facilitate the Preceptor training for the Branch.
- DUTIES AND RESPONSIBILITIES- Using the principals of adult learning, the Practicum Educator will teach CHW students using a variety of methods including lectures, demonstrations, discussion groups, interactive activities, independent and group projects, and peer mentorships
- Make recommendations to the Area Director regarding job suitability of CHW students
- Participate in program evaluation
- Facilitate train the trainer Preceptor Training Program.
- Organize and implement a preceptorship plan for each student
- Evaluate progress of students once they have completed core course components, and make recommendations
- Stay informed about changes and innovations in Healthcare and the PSW field, and with approval; make the appropriate changes to curriculum
- Maintain student files
- Connect with appropriate stakeholders
- Provide timely feedback to students regarding academic progress
- Perform other tasks as required

SPECIFIC RESPONSIBILITIES - see attached detail role
- Theory- Receives list of CHWs for the branch
- Attends and facilitates all orientation days scheduled weekly during theory (team meetings)
- Ensures students have completed all components of orientation to Bayshore including HR orientation as well as clinical orientation
- Works with HR as required- Provides support to IBT instructor re issues and concerns
- Supports appropriate CHW access to the databases (Procura, PeopleSoft)
- Arranges PSW preceptors for all CHW students
- To be complete prior to student finishing their theory- Support the completion of the CPR requirement through St Johns
- Provides copy of CPR certification to IBT- Provide specific hands-on training following the theory to prepare CHW for practicum. This will be a 2-day session
- Practicum- Provides overall support, supervision and evaluation to CHWs who are completing their 80 hr practicum
- Required to attend on site (face to face) at the client’s home to provide supervision as well as phone support
- Ensure student has opportunity to observe and demonstrate skills with a variety of clients.
- Monitors student’s progress in the Skills Passbook and updates Passbook at least weekly
- Works with student and preceptor to complete the ongoing evaluation of skills- Provides support to preceptor where conflict arises and can’t be resolved or where there are issues related to attendance, performance etc
- During practicum, the Practicum Educator will visit the student with the preceptor in the client’s home 2 times weekly initially and then at least weekly ongoing
- Practicum Instructors will also arrange regular phone touchpoints with the student and preceptor to ensure there are opportunities for completing the required skills
- Collaborates and supports the CHW creation of their learning plan for the post practicum period
- Post Practicum 3 Months Period- Provides ongoing support to CHW for 3 months following completion of practicum
- Month 1 - provides weekly contact
- Month 2 - provides bi weekly contact
- Month 3 - provides one contact- With the Managing Supervisor, completes Performance Review and review of Learning Plan before final transition of CHW from Practicum Educator to Manager

REPORTING RELATIONSHIPS
The Practicum Educator will report directly to the Branch Area Director and work closely with the Senior Clinical/Education Specialist who is responsible for CHW program.
