Reporting to the Director of Medicine, in collaboration with the Director, Quality Improvement & Risk, the Clinical Quality Improvement Lead will work closely with the Medicine Program Medical Director and Chief, leadership team and staff to integrate quality improvement across the Medicine Program at Lakeridge Health to achieve the goal of excellence in the delivery of services/care to progress Lakeridge Health's goal of achieving Whole System Quality.

In this role, you will:

- Facilitate the knowledge and skill building with medicine program team members in areas of quality improvement in alignment with approaches used by the central Quality Improvement & Risk team;
- In partnership with Program Leadership and Interprofessional Teams, develop quality improvement initiatives across the patient care areas to enhance patient care outcomes and improve performance metrics;
DUTIES AND RESPONSIBILITIES:

- Project Lead for the development of project and process improvement in alignment with strategic plans including research, data analysis, stakeholder engagement, environmental scanning, and issues analysis;
- Medicine program accreditation leadership;
- Facilitates the implementation of quality improvement activities, track status, and acts as a knowledge expert and resource in the development and maintenance of quality improvement processes;
- Assist and collaborate with Clinical Practice Leaders with the delivery of appropriate education, mentoring and coaching in support of quality improvement methodologies;
- Contributes to the ongoing collaboration, support and evaluation to the development and implementation of Quality Improvement Plans as it relates to the Medicine Program; and
- Other duties as assigned.

QUALIFICATIONS:

- Bachelor's Degree in a Clinical Healthcare field required;
- Membership in good standing within a Regulated Health Discipline;
- Lean Green Belt, Six Sigma Green Belt or equivalent Quality Improvement certificate (preferred);
- Have or in process of Master's Degree in a Health Care program with aspects related to Change management, Informatics, and Quality improvement initiatives;
- Ability to understand and interpret data in order to respond appropriately;
- Knowledge and understanding of performance Metrics;
- 3-5 years recent experience related to quality improvement and/or project management in a heath care service environment (preferred);
- Demonstrated knowledge of quality improvement principles, methods, and tools;
- Demonstrated ability to organize and prioritize work; work well independently
- Demonstrated ability to develop and maintain effective working relationships with a variety of stakeholders;
- Demonstrated ability to effectively train, coach, and mentor teams and leaders;
- Proven ability to facilitate project teams and improvement initiatives;
- Excellent interpersonal, communication (presentation, written and verbal), critical thinking, attention to detail and problem-solving skills;
- Experience working with large, multisite organization;
- Proven ability to act as a resource to various health professionals, Managers, Physicians, and Directors;
- Strong analytical skills including the ability to collect, analyze, share and interpret qualitative and quantitative data;
- High proficiency with the MS Office suite;
