**Reports To**: Supportive Housing Manager

**Position Overview**

**Status**: Part-time (20 hours per week), with possibility for blended full-time hours with a Supportive Housing Worker position.

**Requirements**: A valid drivers license, insurance, and a reliable vehicle are required for the role.

The Community Transition Coordinator supports individuals transitioning from homelessness to stable housing. The role involves helping residents develop the necessary skills and access the resources required for successful housing retention. The Community Transition Coordinator ensures that residents are connected to case management, community services, and other supports to facilitate a smooth transition. By offering support and advocacy, the Community Transition Coordinator empowers residents to achieve long-term housing stability and self-sufficiency.

**Key Responsibilities**

**Resident Assessment and Support**:

- Conduct thorough assessments with residents to understand their goals, barriers, and needs.
- Develop individualized support plans that address housing, employment, health, and other personal goals.
- Regularly meet with residents to assess progress, provide guidance, and offer support in achieving success in supportive housing and/or working toward housing independence.

**Case Management**:

- Work closely with Supportive Housing Workers and the Supportive Housing Manager to ensure residents are connected to necessary community resources, including healthcare, legal, employment services, and financial assistance.
- Provide ongoing case management services, ensuring that residents have the support needed to reach housing independence and maintain long-term stability.

**Housing Retention and Skills Development**:

- Assist residents in identifying ways to develop essential life skills, such as budgeting, healthy coping mechanisms, meal planning, home maintenance, and managing household responsibilities.
- Provide residents with tools and resources to effectively manage their housing, reducing the risk of eviction and promoting long-term housing stability.
- Collaborate with residents to identify strategies for overcoming barriers to housing retention.

**Crisis Management and Advocacy**:

- Act as a liaison between residents and community services, advocating for residents when necessary.
- Provide crisis intervention for residents experiencing setbacks, such as substance use relapse, mental health crises, or housing instability.
- Ensure residents are supported through difficult moments, offering solutions and connecting them with the necessary services.

**Documentation and Reporting**:

- Maintain up-to-date and accurate resident files, tracking progress and identifying areas for further support.
- Prepare and submit regular reports on resident outcomes, program effectiveness, and other relevant data.
- Ensure that all documentation complies with regulatory requirements and organizational policies.

**Expected Competencies and Skills**

**Case Management Experience**: Knowledge of case management practices and experience in connecting individuals to community resources and services.

**Crisis Intervention**: Ability to respond calmly and professionally to crises, providing appropriate support and solutions for residents in distress.

**Cultural Sensitivity**: Strong ability to work with individuals from diverse backgrounds and understand their unique needs, demonstrating empathy and respect.

**Strong Communication Skills**: Ability to communicate effectively both verbally and in writing, with clients, staff, and external partners, maintaining clear and open communication at all times.

**Problem-Solving Skills**: Ability to assess situations, identify challenges, and develop creative solutions to assist residents in overcoming barriers to housing and independence.

**Time Management**: Ability to manage a caseload and prioritize tasks to ensure the timely and effective delivery of services to residents.

**Collaboration and Teamwork**: Work effectively within a multidisciplinary team to coordinate services and support residents' transition needs.

Pay: $23.00-$28.00 per hour
