From our humble beginnings in 1951 as a group of mothers going door-to-door collecting dimes in support of polio research, March of Dimes Canada has grown into a national, multi-service not-for-profit organization. We work for people with disabilities and seniors, helping them with daily living, rehabilitation and employment.

**What does the Community Support Services team do?**

Our Community Support Services team, which is primarily funded by governments, provides essential services that enable people to live as independently as possible. Whether the disability is physical, cognitive or the result of brain injury or aging, our team of 1,600 employees assists people in their homes and communities, helping them gain a greater level of functioning and connection with others. Over the past year, the number of people we serve rose to more than 6,100 people.

**This Opportunity**

We are currently looking to fill a **Full-Time** **Permanent**, **Community Case Coordinator** (35 hrs weekly) role in our Northeast Region, **Sudbury ON.** (1 position), and **Sault Ste. Marie ON.** (1 position). These positions will be part of our Neighbourhood Model of Care programs and referenced within the locations as a **Tenant Wellness Coordinator.**

**Responsibilities include the following**:

- Conduct assessments to determine identified tenant health and social service needs.
- Provide information and support referrals to external community resources.
- Act as a liaison between tenants, their care team, and other health agencies as needed.
- Provide tailored person-centered support for tenants including systems navigation, care coordination, communication, and collaboration with the tenant’s circle of care to ensure best outcomes for tenant wellness.
- Highly organized and excel at keeping client files and information accurate, complete and confidential.
- Research, identify, implement, promote and evaluate wellness initiatives that support a healthy, resilient and engaged tenants.
- Develop and implements an early intervention program to identify tenants who may need support and assistance and encourage tenants to take an active role in their health a wellness needs.
- Contribute to the planning, monitoring, review and evaluating of Neighbourhood Model of Care programs/services for continuous quality improvement.
- Flexible to the changing demands of the job, manage workload and work area in a healthy and safe manner, and manage change in a constructive and positive fashion.
- Provide excellence in customer service, diplomacy, conflict resolution and problem solving are key skills required in this position.
- Work safe according to WHMIS, Occupational Health & Safety (OH&S) laws, regulations, MODC policies and procedures.

**Qualifications and Assets**:

- Post secondary diploma or degree in a health/social science (social work, nursing background preferred)
- Experience in community health planning and/or health promotion
- Experience working with older adults, adults with age related health conditions, and/or disabilities.
- Excellent organizational and problem-solving skills.
- Excellent communication (written and oral) and interpersonal skills.
- Attention to detail, accuracy, and organization skills.
- Knowledgeable with MS-Office software (word, excel, outlook)
- Demonstrated ability to work alone and in a multi-disciplinary team.
- Energetic, outgoing and a passion for health and wellness.
- Proficiency in health program development, design, delivery, and evaluation.
- Experience with group facilitation is considered an asset.
- Highly collaborative and work well with a variety of work/personality styles.
- Clear CPIC-vulnerable person check
- Valid class G driver’s license and a reliable personal vehicle.
