This position reports to the Supervisor of Toronto Public Health's Toronto Urban Health Fund.
Major Responsibilities:
- Assist in conducting community consultation and other relevant action necessary to develop and recommend funding priorities for programs to respond to community needs, funding policies/programs.
- Assist in the collection of data and information for Board of Health reports. Analyses and makes recommendations based on current trends and the projection of future needs.
- Coordinates outreach and consults with community groups for the development of innovative projects to increase ability to address community needs.
- Develops and delivers training and presentations to groups and funded project staff regarding grants, proposal development, funding issues, capacity building, other matters related to funded services and city policies.
- Responds to inquiries about funding program criteria and practices in order to address client and community questions and build public understanding of the programs and processes.
- Responds to inquiries from the public and media related to the grants program.
- Conducts thorough reviews and assessments of community project proposals for funding based upon best practices and evidence, community health needs, groups capacities and project grant criteria, including a review of financial statements to ensure that providers are operating in a fiscally and programmatically responsible manner.
- Prepares contractual agreements and correspondence such as approval letters, legal agreements, and trustee agreements.
- Recruits community members and establishes community review panel committees.
- Makes annual recommendations and contributes to the briefing note for the Medical Officer of Health
- Makes recommendations to a review panel and the Board of Health for the release of grant funds and requests for modification of the contract's project activities to approved agencies.
- Ensures that funded agencies are in compliance with contract requirements and/or other similar agreements, standards, guidelines and relevant regulations.
- Monitors service outcomes and compliance of contracted non-profit agencies by conducting regular on
- site visits.
- Supports the community development work of front-line public health staff so that they can liaise with defined communities (defined by ethnicity, geography, age, etc.) in the promotion of the grants program.
- Develops/maintains a database of program information and the complete files on each project.
Key Qualifications:
- Baccalaureate degree in Health Promotion, Sociology, Social Work or Community Health, or approved equivalent combination of education and/or experience working in these areas.
- Considerable experience working with marginalized priority communities to address health disparities and the social determinants of health.
- Experience with community grants and the various stages of the grant management process.
- Considerable experience in community development, capacity building, and health promotion
- Considerable experience developing and facilitating workshops and training.
You Must Also Have:
- Excellent oral and written communication skills, including the writing of reports, briefing notes and project summaries.
- Proficiency working with Excel spreadsheets and data reporting programs.
- Familiarity with the sociology of a large and complex urban area with an ethno-racially, linguistically, culturally, and economically diverse populations including issues facing urban Indigenous populations, racialized populations and cultural groups.
- Strong project management skills and ability to prioritize efficiently and effectively under time constraints.
- Ability to work independently and as part of a team.
- Ability to support the Toronto Public Service values to ensure a culture that champions equity, diversity, and respectful workplaces.
- Ability to work flexible hours which may include overtime and occasional evenings and weekends.
