Social Worker Community Integration Program - Assertive Community Treatment Team

As a member of the interprofessional team, the Social Worker leads the teams in the engagement and partnership with

individuals and their family members or their natural supports, in the treatment/service planning process, and in individual

and/or multiple family support therapy. In addition, expertise is provided with respect to entitlements, advocacy and

system navigation. The Social Worker also contributes to the development of the comprehensive client-centred

assessments and service plans and provides service coordination for an assigned group of individuals that includes

providing a range of treatment. Rehabilitation and support services.

**Role Responsibilities**:

- Provides ongoing assessment of individuals’ mental illness symptoms and individual’s response to treatment. In

collaboration with the individual and the interprofessional team makes appropriate changes in treatment and
recovery plan to ensure immediate and appropriate interventions are provided in response to changes in mental

status or behaviour, which put individuals at risk (e.g. suicidal).**:

- Provides individual and group assessment and treatment in a stage-based treatment model that is non-confrontational; considers interactions of mental illness and substance abuse, and has individual-determined goals
to address issues related to concurrent disorders**:

- Collaborates with the interprofessional team, to assist in the organization and management of the system of

medication administration and integrates medication administration into individual recovery plans**:

- Provides ongoing assessment, problem-solving, side-by-side services, skill training, supervision (e.g. prompts,

assignments, monitoring, encouragement), and environmental adaptations to assist individuals with activities of
daily living**:

- Promotes leisure activities and hobbies based on each individuals’ interests, by organizing and leading individual

and group social and recreational activities to structure individuals’ time, increase social experiences, and

providing opportunities to practice social skills and receive feedback and support**:

- Provides individualized psycho-education to family members and other major supports about individuals’ illness

and their role in the therapeutic process**:

- Contributes to the planning for and provision of work-related supportive services, such as assistance with

grooming and personal hygiene, securing of appropriate clothing, wake-up calls, and transportation consistent
with the individualized placement support model**:

- Provides clinical opportunities and education for Social Work students**
***Required Qualifications and Experience**:

- Master’s Degree Social Work
- Unrestricted registration with the College of Social Workers and Social Service Workers of Ontario
- One-year work experience providing services to adults with a serious mental illness in an outpatient psychiatric

setting, effectively accessing and employing community resources to meet the need of the individual preferred
