**Job Summary**:
The Transitional Rehabilitation Housing Program Social Worker reporting to the Program Manager, works as a part of an
interdisciplinary team providing psychosocial rehabilitation and person-centered support to individuals within the
Transitional Rehabilitation Housing Program. The Transitional Rehabilitation Housing Program Case Manager works
independently and in conjunction with partners, carrying out treatment rehabilitation and support functions to facilitate
successful community living for Providence Care Forensic Mental Health clients under the jurisdiction of the Ontario
Review Board (ORB), using a recovery oriented model of care for clients within the Transitional Rehabilitation Housing
Program. The Transitional Rehabilitation Housing team member aims to build the capacity and strengths of individuals
and works as an integral member in building a sense of community within the residential setting. This incumbent is
responsible for participating in the comprehensive assessment of clients, coordinating the individualized treatment plan
and required services and needs of assigned clients and their informal supports. Additionally, the incumbent is responsible
for providing rehabilitation support to these individuals, including but not limited to assistance with instrumental activities
of daily living(IADL), daytime activities, and sharing in the responsibility for addressing the needs of all clients providing in
home support and supervision. Social Work expertise is shared with other team members to assist in addressing the health
and well-being of clients and to assist in each client’s engagement and partnership with family members, accessing
entitlements, and individual advocacy to ultimately support recovery. All services are provided in accordance with the
mission and established protocols of the Agency and the principles of psychosocial rehabilitation.

As such, the Case Manager for French Language Services is responsible for providing services to clients in English and
French and works collaboratively within the team on supporting French-speaking clients outside of their assigned case
load. The incumbent may also be asked to support French-speaking clients of other programs of the agency as required.
Travel may be required to all KFL&A sites.

**Note**: If the competition is unsuccessful for a French Language Services individual, applicants without proficiency in French

**Minimum Qualifications: -**
Oral French Language proficiency at the Advanced level, in accordance with the provincial standard. Documentation

of proficiency and/or additional third-party testing/assessment is required.
- Bachelor of Social Work and current unrestricted registration with the Ontario College of Social Workers and Social

Service Workers.
- Three years relevant experience providing services to individuals who live with a serious mental illness, preferably in

a community setting.
- Certification in First Aid and CPR.
- Applied Suicide Intervention Skills Training an asset. Crisis Prevention & Intervention training an asset.
