Resident Health Coordinator

**About the Role**
The Resident Health Coordinator (RHC) plays a vital role in supporting resident health, safety, and care coordination in a retirement living environment. This position supports clinical oversight, care planning, documentation, infection prevention, and communication with families, staff, and external partners — all while maintaining a strong resident-centered approach.

**Key Responsibilities**
- Coordinate and support resident care, documentation, and care plan reviews
- Review daily EHR and medication records; identify and escalate concerns
- Ensure vitals, diabetic monitoring, and ordered care tasks are completed and documented
- Support infection prevention and control (IPAC) audits and outbreak readiness
- Maintain accurate census, signage, and care tracking tools
- Communicate with families, physicians, pharmacies, and leadership as required
- Support medication ordering and supply coordination
- Conduct room and environmental audits as needed
- Answer resident call bells and maintain a visible presence in the home
**Scheduling & Administrative Support**
- Assist with staff scheduling, availability tracking, and payroll preparation
- Coordinate resident move-ins and move-outs with Head Office and pharmacy
- Support care conferences and monthly documentation requirements
- Ensure resident and employee files meet regulatory and Trillium standards
**Qualifications**
- Experience in retirement living, long-term care, or healthcare coordination
- Strong working knowledge of Electronic Health Records (EHR) and digital care documentation
- Ability to confidently navigate web-based systems, dashboards, and internal portals
- Comfortable reviewing, entering, correcting, and validating data within digital systems (care plans, medication records, vitals, reports)
- Able to interpret digital reports and summaries and identify missing, incorrect, or outstanding information
- Comfortable using online forms, checklists, and workflow-based systems
- Strong attention to detail and understanding that accuracy is critical for compliance and resident safety
- Able to follow structured digital processes and adapt to system updates or changes
- Strong organizational, communication, and time-management skills
