The Care Supervisor ensures quality of client care by supervising Community Health Workers (CHWs) and Registered/Licensed Practical Nurses (RPN/LPN) in the Vancouver and Victoria regions, performing routine community visits, working collaboratively with staff and other care professionals involved in the provision of care. The Supervisor performs duties within the competency guidelines and standards of practice outlined by the BC College of Nursing Professionals.

**Key Duties and Responsibilities**:

- Supervises staff including providing ongoing feedback and conducting probationary and periodic performance reviews
- Participates in orientation, training and performance management of staff
- Maintains and fosters effective and timely communication with care coordinators and staff to ensure effective day to day operations
- Communicates with clients, caregivers and families regarding care needs, conducts client visits and updates care plans as needed
- Provides staff with consultation and training for delegated tasks, where applicable
- Conducts client assessments, oversees care plan delivery, and communicates client care plans to staff and care coordinators
- Documents, investigates, and responds to inquiries, client feedback and concerns related to the delivery of care services by investigating practice issues and taking required and timely action
- Assists with WorkSafe BC accident investigation follow-up as required including home visits to review and ascertain events that led to the accident. Reports information verbally and in writing to Chair, OH & S committee
- Maintains pertinent electronic and records as required and participates in data collection activities for quality reports
- Provides backup for other supervisory staff during absences
- Participates in rotating on-call schedule
- Follows employee relations processes and all applicable policies and procedures
- Other reasonable duties as assigned

***Qualifications**:
**Education, Training and Experience**:

- Full scope RPN Certification and current BCCNP registration in good standing required.
- One-year supervisory experience in a health care setting.
- Current CPR certificate and immunization record including COVID-19 vaccines
- Vulnerable sector check current within the past year

**Skills and Abilities**:

- Communicate effectively both verbally and in writing
- Supervisory experience and skills
- Independent with a minimum of supervision
- Ability to establish and maintain good working relationships, and to participate effectively as a team member
- Ability to recognize and respond to changes in client needs.
- Physically and mentally able to carry out the assigned duties in the working environment
- Must have a valid driver’s license and access to a personal vehicle
