Behavioural Support Coordinator

**Position Summary**:
- Works in a team environment in efforts to assist with coordinating the care and services for residents with responsive behaviors.
- Responsible for promoting and contributing to the development of a person-centered philosophy of care for residents exhibiting challenges and utilizes effective communication and interpersonal skills to collaborate with the resident, family, staff and care providers to ensure that a holistic plan of care is in place for the resident.
**Qualifications**:
- Must possess current Registration from the College of Nursing.
- Must possess a Registered Practical Nurse Diploma.
- Experience in long-term care preferred.
- Proficiency in interpersonal communication skills, both written and verbal.
- Demonstrates knowledge in dementia care, delirium, mental health issues and the impact that is has on the resident’s quality of life, family members and care partners.
**Reports to**:Programs Manager and Social Services
- MAJOR DUTIES AND RESPONSIBILITIES_
- Utilizes effective communication and interpersonal skills when interacting with the resident, family members and internal and external care partners.
- Delivers person
- centred care which recognizes both the uniqueness of each resident and an awareness of ones’ own contribution to the relationship, including personal attributes, values, and action.
- Uses assessment tools based on best practices to conduct initial and ongoing assessments describing interventions with respect to the behaviours of the residents including:
Ø Documenting observational behaviours, recognizing that they have meaning, assessing and accounting for how the behaviours impact the care delivery process
Ø Assesses the meaning and identifies the behavioural risk using objective, systematic and holistic process that take into account the P.I.E.C.E.S.
Ø Identifies caregiver strategies that are resident-centred, age appropriate, maintains the resident’s when responding to the behaviours and managing the associated risks.
- Conducts behavioural rounds with the interdisciplinary team.
- Ensures that there is a current plan of care in place for the resident with responsive behaviours that directs the staff in providing care and mitigating risk to the resident, other residents, staff and care providers.
- Liaisons with external partners to support resident-centred care.
- Communicates to senior management staff any areas of risks associated with responsive behaviours.
- Participates in quality improvement activities and takes part in reporting monthly stats on responsive behaviours.
- Acts as a mentor and coach to staff to ensure collaboration of care and services for responsive behaviours
- Conducts resident case reviews to support the plan of care for the responsive resident.
- Demonstrates an understanding and commitment to the philosophy, objectives, and policies of the Home and the Behavioural Support Ontario Project.
- Completes all tasks, duties and documentation with efficiency and professionalism
- Works flexible hours to meet behavioural needs of the residents exhibiting responsive behaviours
**Salary**: $62, 000 - $68, 000
Schedule:
- Monday to Friday
- Weekends as needed
**Education**:
- Bachelor's Degree (preferred)
Licence/Certification:
- First Aid Certification (preferred)
- LPN/RPN (required)
- Nursing (required)
