**Job Description**:
In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority (Island Health) patient and staff safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.
Under clinical supervision of the Community Support Team Leader, provides ongoing intensive outreach nursing care and case management to the most seriously mentally ill adults in a specific geographic region. Designs, implements and monitors individual treatment plans to maximize community functioning and reduce re-hospitalization. Works in close association with multi-disciplinary community mental health staff, mental health agencies and community service providers
**QUALIFICATIONS**:
**Education, Training And Experience**:
Registration with BC College of Nurses and Midwives as a practicing RN registrant or a practicing RPN registrant. Courses in psychosocial rehabilitation preferred. Minimum 2 years psychiatric nursing experience in psychosocial rehabilitation programs. Several years experience in clinical work with chronic mentally ill patients. Strong motivation to develop and practice acquired rehabilitation skills and learn new techniques.
**Skills And Abilities**:
- Excellent communication skills.
- Ability to work independently in an unfamiliar environment.
- Clinical skills dealing with chronic mentally ill.
- Possess a valid Class 5 BC Driver's License.
- Business/Liability Car Insurance.
