Job description

Pursuit Health Management is a dynamic rehabilitation firm located in London, Ontario. We specialize in assisting individuals with their recovery following accidents, illness or injuries. We have a multi-disciplinary clinic in London, offering physiotherapy, massage therapy and a variety of specialized assessments.

Our community team, comprised of occupational therapists, nurse case managers, social workers, kinesiologists and vocational rehabilitation consultants work with clients in their home, school or place of employment, throughout Southwestern and Midwestern Ontario. We’re a team focused on function by meeting the client’s physical, cognitive, and psycho-social needs in a timely and professional manner.

We are currently seeking a part-time Social Worker in the **Owen Sound, ON. area. (Grey -Bruce)**

**Responsibilities**:

- Assess clients in their homes to develop a treatment plan
- Provide one on one counselling to address psycho-social issues arising from injuries sustained or illness
- Work as a member of a healthcare team, collaborating with medical, community, insurance and legal stakeholders
- Comprehensive knowledge of community resources to further support client’s recovery
- Provide system navigation services as required to fully meet the needs of clients
- Support client and family through the recovery process
- Provide written reports, detailing client’s recovery and outlining future treatment goals
- Practice is reflective of evidence based, goal oriented and client centered approach

**Qualifications**:

- MSW Degree
- Current registration with the College of Social Workers and Social Service Workers
- Minimum of two years of clinical/community based experience
- Excellent people skills
- Highly developed communication skills, both written and oral
- Self-motivated
- Clinical skills in the area of trauma, PTSD and family dynamics
- Experience with motor vehicle accident clients an asset
- Ability to work collaboratively with a multi-disciplinary team
- Valid driver’s license and use of a vehicle with ability to travel
- Can work independently in a flexible environment
