The Alzheimer Society of Niagara Region (ASNR) is a community support service organization that provides programs and services to people living with dementia and their care partners in the Niagara Region. We work closely with other service providers to ensure quality care and support for our clients.

We employ administrative and fund development staff, educators, social workers, nurses, therapeutic rec staff, personal support workers and volunteers who work together as a team to meet the needs of people living with dementia and their care partners.

The Wellness and Early Stage Program Coordinator is responsible for the planning, implementation and evaluation of the Early Stage programs at the ASNR. The Early-Stage Program Coordinator works closely with persons living with dementia and supports their meaningful engagement in participating in Early Stage programs such as, but not limited to, Beyond Words Early Stage learning series, Advisory Group, Early Stage Wellness Program, ROPES and the R.E.B.E.L. group through an authentic partnership approach.

**Position Description**

**Title: Wellness and Early Stage Program Coordinator (18-month Maternity Contract)**

Reporting To: Director of Education

Hours: 40 hours per week (paid lunch break); Monday to Friday - 8:30 am - 4:30 pm

Salary Range: $42,000 - $48,000

Vacation: 3 Weeks paid vacation to start

**Duties and Responsibilities**

Responsible for the planning, implementation, management and evaluation of Early Stage programs including but not limited to: Wellness Early Stage Program, Minds in Motion, Power of Music, Beyond Word Learning Series, Learning the ROPES for living with MCI, Rebel Group, Client Advisory Group, Sole Mates and Connections First Education Program.
- In conjunction with the Director of Education and best practices, develop appropriate educational and promotional materials related to Early-Stage programs.
- Communicate with community members who have questions regarding Early-Stage programs or their experience with dementia.
- Receive/review all referrals to the Early-Stage programs.
- Contact individuals and care partners referred to the early-stage programs and schedule interviews to discuss the particular program.
- Obtain appropriate consent from all parties in accordance with best practices and ASNR policies.
- Regular communication with the Coordinator of Volunteer Programs regarding volunteer engagement, reporting of volunteer statistics and concerns or challenges that may arise
- Create and distribute early-stage newsletter - Connections
- Participate in appropriate internal and external committees
- Attend Meetings as appropriate.
- Execute administrative function related to the education department, including but not limited to collecting and reporting statistics and program evaluation.
- Complete required documentation/data entry in AlayaCare Database in a timely manner Maintain a current knowledge of Alzheimer’s disease, related dementias and current resources. Be prepared to upgrade skills as required.
- Work collaboratively with staff, volunteers and community partners to meet the Society’s mission
- Research opportunities for community partners to participate in Early Stage programs and contact each partner to organize their participation
- Supervise student engagement and participation and ongoing follow-up with the student’s supervisor in this work environment and with the student’s faculty advisor
- Assume other functions as may be assigned by management
